Job Overview
We are seeking a compassionate and dedicated Community Support Worker to join our homecare team. This role involves providing essential support to patients across various settings, including community settings. The ideal candidate will have the ability to work well independently or in a group. Responsibilities
- Assist individuals with daily activities such as bathing, dressing, and mobility to promote independence and comfort
- Administer basic medical treatments under supervision, including medication administrations
- Coordinate with team members to implement individualized care plans
- Provide emotional support and companionship
- Document patient interactions accurately in accordance with NLR protocols
- Assist with transportation to medical appointments or community resources when necessary
- Maintain a clean and safe environment
Experience
- Prior experience in caregiving is highly desirable, but not required
- Strong interpersonal skills with the ability to communicate effectively with individual's, their families, and NLR teammembers
- Compassionate attitude with a commitment to providing high-quality community-based support
