The Patient Navigator will lead the development of a Patient Navigator Hub as a SHS central resource for new and ongoing learning and development in patient navigation skills. Establish a therapeutic relationship with patients and families, providing contact to link the patient, health team, hospital, and community services to support care. Advocate for and assist clients through the health care system and address issues as able. Engage with clients to determine medical and nonmedical barriers to care. Liaise on behalf of clients
Education & Experience
- Bachelors degree in nursing or social work or other health related field.
- Current registration in good standing with CRNA or ACSW
- Certification in Patient Navigation or willingness to become certified as a condition of employment
- A minimum of 3 years direct experience working with Indigenous Peoples in a hospital or community based case management position, public health or social work setting.
- Formal Training in case management
- Superior time management skills, multitasking skills and the ability to prioritize tasks with minimal supervision
- Professional, responsive and positive work attitude is essential
- Resourcefulness and flexibility
- Previous experience in handling confidential or sensitive information; knowledge of applicable data privacy laws
Other Requirements
- Valid driver’s license (class 4 preferred)
- Recent drivers abstract
- Criminal record clearance.
- Awareness of Indigenous languages and cultures (an asset).
