The Role
We're looking for a passionate, registered Physiotherapist to deliver high-quality, client-centred physiotherapy services directly in clients' homes across the Greater Toronto Area. This role is ideal for a PT who values autonomy, meaningful work, and being part of a team that genuinely believes in what it's doing.
You will conduct assessments, develop individualized treatment plans, and provide hands-on rehabilitation to clients recovering from surgery, managing chronic conditions, or working to maintain their mobility and independence. You'll be supported by our operations team and AI-assisted platform — meaning less paperwork, more patient care.
Key Responsibilities
- Conduct comprehensive physiotherapy assessments in clients' homes
- Develop and implement individualized, evidence-based treatment and rehabilitation plans
- Provide direct physiotherapy interventions: therapeutic exercise, manual therapy, mobility training, fall prevention, pain management, and more
- Write clinical notes and progress reports in compliance with CPO standards (supported by our voice-to-document AI drafting tool)
- Collaborate with the broader care team including OTs, nurses, and PSWs
- Supervise Physiotherapy Assistants (PTAs) and Resident PTs as applicable
- Communicate regularly with referring physicians, OHAH care coordinators, and family members
- Educate clients and families on home exercise programs and safe movement
- Participate in discharge planning and ongoing care plan review
What You Bring
- Registration in good standing with the College of Physiotherapists of Ontario (CPO)
- Experience in community, home health, or geriatric physiotherapy (preferred)
- Strong clinical reasoning and assessment skills
- Ability to work independently with minimal direct supervision
- Excellent interpersonal and written communication skills
- Comfortable with mobile and digital tools (tablets, EHR systems, etc.)
- Access to a vehicle and a valid Ontario driver's licence
- CPR/First Aid certification
