Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role involves administering medications, monitoring patient health, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. Candidates with experience in acute care, dementia care, pediatrics, hospice, and senior care are highly valued. The LPN will utilize their expertise in medical terminology, phlebotomy, and case management to deliver comprehensive nursing services in a professional and supportive manner.
Responsibilities
- Provide direct patient care in diverse settings such as nursing homes, hospitals, assisted living, hospice, and home care.
- Administer medications accurately and safely according to physician orders.
- Monitor vital signs and assess patient conditions regularly to identify changes requiring intervention.
- Assist with procedures including phlebotomy and pain management techniques.
- Maintain detailed documentation of patient care using electronic health records systems such as Epic and Cerner.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and implement individualized care plans.
- Educate patients and their families about health conditions, medication regimens, and post-discharge instructions.
- Ensure compliance with all healthcare regulations and safety protocols to promote a safe environment for patients and staff.
- Support patients with dementia, pediatric needs, senior care, or hospice requirements with compassion and professionalism.
- Participate in ongoing training to stay current with medical practices and technology relevant to nursing care.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.

- Prior experience in nursing homes, hospitals, acute care, dementia care, pediatrics, or home health preferred.
- Knowledge of medical terminology and medication administration protocols.
- Experience working with electronic health records systems such as Epic or Cerner is a plus.
- Strong communication skills and the ability to work effectively within a multidisciplinary team.
- Ability to handle sensitive situations with empathy and professionalism.
- Certification in phlebotomy or laboratory experience is advantageous but not required.
