Location: Owen Sound
Hours of Work: 12hr Shifts, 84 Hours Bi-weekly. Rotation and every other weekend.

Support Worker’s role in CLOSD’s Intensive Supports program is to recognize the value that every individual contributes to our community and to making each day meaningful. The Support Worker will have the ability to provide life skills support at varying levels depending on the individuals’ needs, encourage life-long learning; along with promoting positive self-worth is the everyday focus. Reporting to the Program Supervisor, this position is responsible for promoting individual safety, security, dignity and respect to all people. Within the scope of this position provide support, assistance and advocacy based on the needs and abilities of each person, in offering them opportunities to participate and share in community life as equal and valued partners. Effective and accurate written and oral communication is required. The Support Worker will demonstrate professionalism as a liaison between people supported, their families, support networks and community partners if required. The Support Worker will participate as a team member by contributing input for the development, implementation and evaluation of Individual Support Plans and day to day living.
RESPONSIBILITIES:

Work each day towards supporting people based on the mission, vision and values of Community Living Owen Sound and District to live their best lives;

Establishing respectful relationships with people, recognizing their rights as citizens; promoting their dignity and well-being;

Support each person as indicated by the person in their Individual Support Plan, Positive Behaviour Support Plan and any other protocols developed by the person supported

Ability to make decisions and exercise judgement within generally prescribed procedures, instructions and standards.

Assist people to develop informed decision- making skills and exercising of choices

The ability to be flexible in adapting to and working effectively within a variety of situations, and with various individuals or groups. Being adaptable in understanding and appreciating different and opposing perspectives on an issue or situation, adapting one’s approach as the requirements of a situation change.

Counselling, behavioural interventions, conflict resolution, money management and interpersonal skill development.

Carry out established supports required for the health, safety and rights for the protection and promotion of each person

Provide support in a person’s home; preparing nutritional meals, administering medication, assisting with personal care and other areas as needed or requested

Demonstrated willingness to collaborate effectively with others is critical to creating alignment within and across groups, and to providing high levels of services to those who are supported.

Maintain stamina and performance and to act effectively under pressure.

Accurately complete any required documentation

Demonstrate a positive work performance.

Must be able to adhere to IPAC (Infection Protection and Control) practices and wear Personal Protective Equipment when and where applicable.
QUALIFICATIONS:
- Completion of a Developmental Services Worker Program/Social Services Worker Program/Personal Support Worker Program and/or minimum 1 -year experience working with people with an intellectual disability.

A good understanding of dual diagnosis.

Team player with an ability to work independently.

Strong communication skills (written and verbal).

Comfortable with web- based technology

Must have a valid G driver’s license and use of a vehicle

Current First Aid and CPR certification. Non-Violent Crisis Intervention certification is required within 30 days of hire.

You must provide a current Vulnerable Sector Check.
