Context
The Transitional Housing Program Caseworker will be responsible for providing comprehensive case management services to individuals and families residing in our transitional housing units. This role involves supporting clients in achieving self-sufficiency through individualized service planning, resource coordination, and advocacy. This role also involves coordinating logistics and communicating with many members of the TRC team and TRC’s broader community network.
Client Assessment and Intake:
Conduct eligibility interviews to assess the needs, strengths, and goals of potential new clients/tenants
Develop service plans in collaboration with clients and the housing team, outlining specific steps and resources needed to achieve eventual permanent housing stability and self-sufficiency.
Case Management and Client support:
Provide ongoing case management services, including regular meetings with clients to monitor progress, update service plans, and address emerging needs.
Assist clients in navigating and accessing community resources such as employment services, housing support and other services, both internally at TRC and externally
Advocate on behalf of clients with service providers and other community agencies to ensure they receive appropriate and timely support.
Support Services Coordination:
Contribute to life skills on topics such as housing rights and budgeting
Coordinate and collaborate with other service providers (internally and externally) to ensure a holistic approach to client support.
Documentation and Reporting:
Maintain accurate and up-to-date client records, including case notes, service plans, and progress reports.
Crisis Intervention:
Provide support and intervention in crisis situations, including conflict resolution
Develop and implement conflict plans as necessary.
Program Development and Improvement:
Participate in program development activities, including the evaluation and improvement of service delivery methods and practices.
Stay informed about best practices and emerging trends in transitional housing and case management.
Qualifications:
Education: Degree/certification in Social Work, Human Services, or a related field.
Experience: Experience in case management, preferably in a housing or emergency shelter services setting.
Skills:
Strong written and spoken communication skills required in both English, & French
Other third languages, such as Spanish, Arabic and Farsi are considered strong assets
Proficiency in English or another language is required for this position in order to communicate with a predominantly English-speaking or allophone clientele.
Strong assessment and counseling skills.
Knowledge of community resources and social service systems.
Knowledge of immigration system and the newcomer population is a strong asset
Ability to work independently and as part of a team.
