Naz's Pharmacy are looking for Licensed Practical Nurses to join the Home Monitoring Program to provide local neighborhoods and communities with excellent pharmacy services.
With more than 200 patients and more than 40 employees, Naz's Pharmacy strives to dispense a healthier lifestyle at home one visit at a time.
We are currently looking to fill full time and part time for Licensed Practical Nurses IMMEDIATELY in our Surrey locations.

Job Type:
- Weekdays Availability.
- Saturday to Sunday Availability.
- Full Time.

- Weekends as needed.
- Statuary holidays.
- Morning/AM Shift: 7:30 am - 1/2 pm.
- Evening/PM Shift: 3 pm - 8/9 pm.
- Each shift ranges between 5 - 8 hours on weekends and weekdays.
Please note that this position requires a driver’s license.
Detail Overview:
- Join the Home Monitoring Program at Naz's Pharmacy and give care to patients within the comfort of their own homes. (Multiple patients per shift).
- Working with Naz's Pharmacy means no ADL's, personal care or transferring patients. Working with us is mainly medication administration focused.
- Work under direction of Pharmacy Management and LPN Lead and within the scope determined by BCCNM and company policies.
- No overnight shifts!
- Be a member of the multidisciplinary team by collaborating with patients, family members, pharmacy and other health care professionals.
- Perform head to toe assessments on patients and plan, implement, contribute to and evaluate nursing care.
- Tend to patient's physiological, psychological, socio-cultural and spiritual needs.
- Create a supportive and educational environment for patients and family to help promote independence in health care decisions and respect choices in their own care.
- Provide nursing care as per pharmacy and physician instructions and as per policies and evaluate outcomes and escalate accordingly.
- Document patient care in Real Time daily (per shift) and update care accordingly. Communicate with pharmacy to ensure that nursing care is relevant and up to date.
- Maintain patient's own documentation in their logbooks to ensure they are, and their close ones, are up to date with current care and health status.
- Maintain a safe and clean environment and report any deterioration in patient's self care.
- Orientate and educate new hires as you become a competent staff member.
- Keep up to date with your continuous professional education and join in on educational sessions provided by management.
- Other duties related.
Skills not limited to:
- Head to Toe assessment: Checking Vital Signs, Weight Management of patients, interpreting results, adjusting medications accordingly and investigating any changes.
- Medication Administration: Oral, Inhalers, Eyedrops and Eardrops, Ointments and Creams, Medical Patch Administration.
- Intramuscular Injections.
- Subcutaneous Injections.
- Diabetes care: Ability to interpret Blood Glucose readings, understand symptoms, follow Insulin Sliding Scales/Algorithms and carry out Hypoglycemic Protocols if necessary.
- Ability to act in times of emergencies accordingly as per BCCNM scope and company policies.
- Mental Health: Checking Medication Compliance and Witnessing Ingestion of Medication. Noting and escalating any changes in behaviors.
- IT Skills: To be able to Electronically Document patients' care in Real Time daily and escalate care via phone call and email to the appropriate personnel. Microsoft Outlook and Google Apps Proficient.
- Organized and have excellent Time Management Skills.
- Physically able to carry out duties.
