JOB DESCRIPTION:

PAY: $31.80 - $35.38

JOB TITLE: COMMMUNITY BASEDYOUTH WORKER

REPORTING TO: CLINICAL DIRECTOR/CLINICAL MANAGER

POSITION SUMMARY:

The Youth Worker will work with youth and families to promote change and growth within the family, home, school and community. They will conduct assessments to determine treatment recommendations. They will provide a combination of individual and group counseling to clients. Family counseling will be provided by the Youth Worker from a practical skills-based approach.

QUALIFICATIONS/CONDITIONS OF EMPLOYMENT:

· CYW Diploma or a recognized equivalent training program such as a degree or diploma in Psychological/Social Sciences

· Must be registered and in good standing with an appropriate professional regulatory college (e.g. Ontario College of Social Workers and Social Service Workers, the College of Registered Psychotherapists of Ontario).

· Training in Crisis Intervention, Risk Assessment, and Cognitive and Dialectical Behavioural Therapies are considered assets.

· Training in Suicide Risk Assessment and Support is an asset.

· Minimum of 1-3 years’ experience in adolescent mental health (is preferred)

· Knowledge of EMHWare electronic client system/ChYMH assessment tools is an asset

· Satisfactory Criminal Reference Check with Board Record Check and Pre-Employment Medical.

· Valid Class G driver’s license with acceptable driving record and experience.

· Demonstrated ability to work independently.

DUTIES AND RESPONSIBILITIES:

Clients

· As a member of a multi-disciplinary team, the Youth Worker will conduct assessments, compile histories and formulate a social diagnosis; this would include the developing an understanding of the personal and environmental causes of behaviour and emotional dysfunction of families and youth. This is essential job responsibility determines the course of treatment for clients.

· To develop treatment plans for clients in service, in conjunction with other services within the agency.

· To liaise with other disciplines, jurisdictions and community resources in delivering treatment plans.

· To work independently on an assigned caseload (mostly individuals) within the framework of a treatment team.

Administration

· To document treatment planning and maintain appropriate agency reports on individuals and families served.

· Ensure that all reports are kept in order and up to date, including medication logs, communication logs, treatment plans and reviews, incident reports etc.

· Attend and participate in clinical meetings

· To provide input into program evaluation and development.

· To case manage clients and have regular contact with clients on the wait list.

· When required, respond to crisis calls in a timely fashion.

The Youth Worker may be assigned the responsibility of supervising students on field placements.

· Ensure that student placements assigned to you are meeting objectives established by the school and agency.

PROFESSIONAL DEVELOPMENT:

· In conjunction with supervisor, participate in regular supervision sessions and an annual performance evaluation.

· Attend and participate in scheduled team meetings.

· Attend and participate in In-Service training as scheduled and required and attend recommended professional development opportunities.

WORKING CONDITIONS:

· Work is performed in various settings (such as office, vehicle, work from home, community).

· Work hours and schedules are set by the Clinical Director/Clinical Manager and evening work may required.

· Requirement to drive and have access to a reliable vehicle.

· May be required to work with aggressive and/or volatile clients
