The ROLE
As a ROCK Care Pathway Clinician, you will focus on early assessment, risk identification, education, counseling, and referral/service coordination of infants and young children up to 8 years and their family members.
As part of our continued growth, we are hiring for a newly created full-time contract position, ending March 31 2027 reporting to the Manager - Counselling and Therapy.
In this role you will:
- Contact and monitor families on the waitlist for therapy services and navigate into appropriate services
- Provide support, consultation, assessment, and navigation treatmentservices to infants and children, primarily under 8 and their families using family, individual, parent-child, and group modalities as required. May also need to service children over the age of 8.
- Conduct in-person sessions and complete formalized screeners and assessments
- Present assessment findings and case synopsis to multidisciplinary team
- Conduct scheduling of initial sessions and warm transfers to service provider
- Remain as a point of contact for families as they proceed through an episode of care
- Review all available assessment reports and incorporate information into the planning process.
- Obtain consents and gather and review other reports (current and historical) for the purposes of informing a client care plan, goals, and treatment strategies
- Collaborate with the caregiver/family, supervisor, and the interdisciplinary team to develop a coordinated plan of careto meet the child and family's needs.
- Provide level 1 service coordination with outside professionals, as appropriate.
- Liaise and advocate on behalf of the child and family to other staff members and to significant professionals and programs in the community.
- Provide clinical support to administrative staff, as requested.
- Document all client interactions (including but not limited to session notes, service summary reports, crisis case notes, referral forms, etc.) in a timely and succinct manner, meeting agency, professional, and legal requirements.
- Ensure that all required program reporting information, including statistical data, is submitted according to set deadlines and procedures.
- Promote the direction of the program and Agencyby engaging in identifying, developing, and implementing program goal(s), projects, or services.
- Supervise the clinical and administrative work of students/trainees.
- Participate in individual and group supervision and related training opportunities.
- Operationalize client safety through adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care, and by identifying and reporting any client safety incidents.
- Commit to supporting client-centered care by adhering to ROCK’s principles of respect, collaboration, and service planning, as outlined in the agency’s strategic plan.
- Abide by the specific requirements of Section 28 of the Occupational Health and Safety Act, as well as any further health and safety responsibilities outlined in the agency's policy and procedure documents.
- Other duties may reasonably be assigned.
Who we’re looking for:
Education:
- A relevant master’s degree from a recognized university.
- Registration with a relevant accrediting professional body is required, i.e., College of Social Workers and Social Service Workers (OCSWSSW), College of Psychologists and Behaviour Analysts of Ontario (CPBAO), or the College of Registered Psychotherapists of Ontario (CRPO).
- A certification in Infant Mental Health and training in infant/child development is preferred.
Experience:
- A minimum of 1-3 years related experience in the areas of assessment and clinical intervention with infants, children and their families, attachment interventions, case-based direct service, groups, and community-based work. It is expected that this experience will have included appropriate supervision.
Skills:
- Strong interpersonal and interviewing skills.
- A demonstrated ability to apply relevant knowledge and training to the assessment and treatment of children and their families.
- Bilingualism is a strong asset.
- Excellent verbal and written communication skills.
What will help you succeed:
- Access to reliable transportation in alignment with program needs.
- Proficient in the use of technology as required of the position with MAC OS operating systems experience preferred.
- Work requires In-person support within our Halton Locations
- Current (within three months) criminal reference check with vulnerable sector search.
- Flexibility to work evenings as required.
- Ability to work on-site (in-office).
