The Carefirst Family Health Team (FHT) is seeking full-time/part-time permanent and locum family physicians to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.
About Us
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, service navigators, dieticians, physiotherapists, and medical receptionists. We have a robust system of Chronic Disease Prevention and Management programs, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our “Hub & Spoke Model” and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with Carefirst Seniors & Community Services Association to provide truly integrated services.

Clinic Locations
We currently operate four well-established clinics across Scarborough, Markham and Richmond Hill, and continue to expand in Markham and Stouffville to support continued growth and community needs.
Physicians will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.

With this rapid expansion, we are looking for compassionate, team-oriented family physicians who are committed to delivering high-quality care and value interdisciplinary collaboration to fill a new position at our Family Health Team.

Qualifications
-
Licensed, or eligible for licensure, to practice family medicine in Ontario and in good standing with the Collage of Physicians and Surgeons of Ontario (CPSO).

Certification in Family Medicine with the College of Family Physicians of Canada (CCFP), or equivalent eligibility, is preferred/required depending on practice arrangement.

Active OHIP billing number

Strong communication, clinical judgement, professionalism, time management and collaborative practice skills

Compassionate, approachable and comfortable working with diverse patients, families and interprofessional team members.

Interest in digital health, quality improvement, teaching, population health, preventive care, or integrated care is an asset.

Ability to provide care in additional languages, such as Cantonese, Mandarin and Tamil, is an asset.
