The John Noble Home requires a Registered Practical Nurse – Multiple Positions

SURROUNDING COMMUNITIES

A growing community of approximately 136,000 residents, the City of Brantford and
Brant County is located on the exceptional waters of the Grand River and is a designated Canadian Heritage area. Brantford has a relaxed community lifestyle, a diverse economic base, and quality residential, educational, and cultural amenities.

JOHN NOBLE HOME BACKGROUND

The John Noble Home (JNH) is a “Class A” facility and has a proud and long-standing care-giving tradition, dating back to 1881. Today, we offer several different quality programs devoted to senior housing, care, and services that operate within our care campus. All meet and serve the needs of both Brant County and the City of Brantford and their respective constituents as we are a municipally operated and governed facility.
The John Noble Home consists of 156 long term care beds with planned expansion, a community-based day & stay program and senior’s apartments.

POSITION SUMMARY (detailed job description available upon request)

The Registered Practical Nurse shall provide professional care as a member of the inter-disciplinary team. The RPN is primarily responsible for the administration of medications and treatments, participation in the assessment and individualized care planning practice. The RPN works collaboratively with the RN and defers to the RN in unpredictable situations. The RPN will direct and supervise Personal Support
Workers. The RPN works in accordance with all the Policies and Procedures of the
Home. While performing any and all duties of the position the employee will work in a safe and healthy manner and will follow the regulations outlined in the Occupational

Health and Safety Act outlined in the organization Health & Safety Policy, Infection
Control and safe work procedures.

KEY RESPONSIBILITIES (but not limited to):

Assumes responsibility for care of residents in accordance with the Regulated

Health Professionals and requests support from RN when required.

Consult with the resident and/or their SDM regarding wishes and needs for care.
Advocate and support the resident and/or their SDM in achieving their care goals.
Administer medications and treatments in accordance with the policies and procedures of John Noble Home.
Participate in the documentation and care planning process.
Provide direct care to the residents, (Assists in completing ADL’s) as required.
Provide feedback on PSW/HCA performance.
Complete the requirement of Quality Assurance and Reflective Practice as determined by the College of Nurses of Ontario.
Assist with the Quality Assurance Program by assuring quality practice and quality care. Monitoring indicators, identifying high risks and ongoing auditing is involved.
Report medication errors and complete Medication Incident Form.

KEY QUALIFICATIONS (but not limited to):

Registered with the College of Nurses Ontario (CNO).
Diploma in Practical Nursing
Geriatric experience is an asset.
Strong communication and leadership skills.

As a condition of employment, candidates must submit a current Vulnerable Sector
Check (dated within the last 6 months) from their local police service before employment can begin. Applicants must also provide proof of a negative result from a 2-
Step TB Skin test (dated within 1 year) before employment can begin.
