We are currently looking to hire a:
Physiotherapist – Community Health Team
1 - Full-Time, Permanent (35 hours/week)
Reporting To: Manager, Interprofessional Primary Care Team
This role will work collaboratively to deliver on SCHC and the University of Toronto’s academic partnership mandate, and as such will work closely with the Vice President, Primary Care Transformation & Academic Partnerships at SCHC and the Associate Chair, Strategic Initiatives and Innovations at the University of Toronto, Department of Physical Therapy.
This position is eligible for status appointment with the University of Toronto.
Purpose:
The Scarborough interprofessional primary care model enhances care for the vulnerable, complex clients living in the Scarborough. SCHC operates with an integrated and interdisciplinary approach. All roles collaborate with and contribute to different teams and programs to support the community’s needs.
As we advance our academic mandate, this role will build out SCHC’s teaching practice and delivery model to train the next generation providers aligned with the Department of Physical Therapy at the University of Toronto’s objectives and strategic plan.

Key areas of responsibility:
Patient Care
The Physiotherapist is a caring and supportive individual who acts as the primary agent in identifying and improving client movement and function. Using clinical expertise and sound professional judgment, the Physiotherapist determines the appropriate course of care based on each client's physical difficulties, whether resulting from illness, injury, disability, or ageing. Central to this role is promoting clients' health and well-being and maintaining their quality of life as much as possible. The Physiotherapist assists in the rehabilitation process by developing and restoring body systems where possible — particularly the neuromuscular, musculoskeletal, cardiovascular, and respiratory systems — while also working to maintain and improve motor skills, balance, coordination, overall endurance, and physical fitness. The Physiotherapist must exercise the utmost discretion and bring exceptional interpersonal skills to every client interaction. Additional responsibilities include the development and ongoing review of individualized treatment programs, as well as the thorough completion and maintenance of client files.
Academic Commitment and Community Access
The Physiotherapist will bring an academic mindset to their practice, playing an active role in the training and professional development of physiotherapy students. This includes developing and implementing a structured model for student placements and fostering a collaborative, supportive teaching environment. Working in close partnership with the Department of Physical Therapy at the University of Toronto, the Physiotherapist will take an innovative and entrepreneurial approach to designing and leading pilot programs aimed at increasing access to physiotherapy services for the Scarborough community. Through these partnerships, the Physiotherapist will help expand the reach of quality physiotherapy care while contributing meaningfully to the growth of the profession and the development of the next generation of clinicians.
Educational and/or Professional Qualifications:
- Minimum 5 years’ experience.
- Graduate of a Post-Secondary program in Physiotherapy.
- License to practice with the Ontario College of Physiotherapists.
- Eligible for membership in the Canadian Physiotherapy Association.
- Education and experience specific to the client populations (e.g. chronic disease, mental health and women’s health) an asset.
- Experience in physiotherapy student education or as a clinical instructor an asset.
- Demonstrated commitment to principles of an anti-oppression and cultural competence framework;
- An interest and commitment to working in a low-income, multilingual, multiracial community;
- Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and radicalized and 2SLGBTQ+ communities;
- Community experience an asset.
- ADP Authorizer status required.
- Commitment to service excellence including continuing education and continuous quality improvement.
- Ability to function within an interdisciplinary team.
- Ability to adapt to and work effectively within a variety of situations, and with various individuals or groups.
- Demonstrates an ability to work in an iterative environment.
- Demonstrated ability to be flexible.
- Demonstrated ability in problem-solving and critical thinking.
- Able to generate new solutions and develop creative improvements.
- Demonstrated ability to assist clients, family members or other clients with concern and empathy; respecting their confidentiality and privacy and communicating in a courteous and respectful manner.
- Ability to speak a second language of our priority populations is an asset.

Remuneration: $41.98 – $48.98
Band: B
