Full job description

Function/Purpose: Develops, implements and provides oversight of programs and activities to provide meaningful activity, social engagement, a sense of belonging and self-worth for residents.

Reports to: Director, Community and Recreation Services

Qualifications:

· Post-secondary diploma, degree or certificate in recreation and leisure studies, therapeutic recreation, kinesiology, gerontology or other related field from an Ontario post-secondary institution or other jurisdiction that is equivalent (LTC)

· Safe Food Handling Certificate

· Smart Serve (if serving alcoholic beverages)

- Positive, team-work attitude and approach to work

- Excellent customer service skills

- Current Police Record Check with Vulnerable Sector Check and TB skin test

- CPR/First Aide

Position Specific Responsibilities: include, but are not limited to:

1. Develop, promote, supervise and evaluate a full range of programs and special events appropriate to the needs and interests of residents (includes entertainment, outings and large events)

2. Facilitate human resources within the department (may include: hiring, shift assignments, training, skill development, oversight, performance, guidance/monitoring/mentoring) for team members and volunteers

3. Assist with annual budget preparation in collaboration with the Director Programs/Programs and Services; meet budget requirements

4. Participates in multidisciplinary assessments; implement individualized and person-centred plans based on each resident’s unique needs

5. Facilitate quality of life surveys, collecting/tracking indicators, audits, analysis and creation of action plans to support compliance and continuous quality improvement

6. Facilitate program attendance in RAI and monthly audits/action plans in Activity Pro

7. Ensure resident’s photographs are taken and uploaded to PCC upon admission

8. Participate in resident care conferences, as required

9. Act as a liaison for the Residents’ Council

10. Participate in program evaluations, as required

11. Responsible for program supplies

Safety Roles and Responsibilities

All staff are responsible for identifying incidents and for taking immediate action to ensure the safety of residents, visitors and other staff. Open disclosure must occur when a safety incident happens, regardless of whether it is unplanned, unintended or a circumstance that could have resulted or did result in unnecessary harm to a resident. Data and feedback from staff, resident, client, families and partners can identify system vulnerabilities to inform quality improvement efforts

Pay: $24.11-$25.86 per hour
