Schedule: Monday to Friday, 08:00-16:00 (37.5-hour work week) + On-Call Rotation
Compensation: $60,000 - $67,000 annually

Compassion Network Home Care is looking for a Clinical Care Coordinator to be our clinical leader on the ground in Edmonton. In this dynamic hybrid role, you aren't leaving nursing—you are using your clinical license and field experience as a superpower to drive our daily operations and ensure top-tier care delivery. You are the clinical authority making the tough calls, while also serving as the operational engine driving our daily schedule across Alberta.

Key Responsibilities
- Clinical Leadership: Conduct in-person intakes, assessments, delegations, and training for Edmonton-area clients. Develop and manage individualized care plans.
- Operational Triage: Field daily inbound communications (calls, emails, texts) from clients and staff, acting as the primary problem-solver for daily operational hiccups.
- Strategic Scheduling: Utilize our scheduling software (AlayaCare) to manage care planning, optimize travel routes, and ensure all shifts are filled seamlessly across our Edmonton and Calgary markets.
- On-Call Support: Provide rotational after-hours, weekend, and holiday on-call coverage to ensure safe service delivery.
Qualifications & Non-Negotiables
- Licensing: Active Licensed Practical Nurse (LPN) registration/license in good standing (Required).
- Experience: Minimum 5+ years of clinical nursing experience. (Previous experience in home care, scheduling, or leadership is a massive asset).
- Tech Mastery: Highly comfortable navigating cloud-based platforms (Google Workspace, Salesforce, AI) and modern communication tools. Ability to adapt quickly to robust scheduling software (AlayaCare).
- Typing Speed: Minimum 60 WPM required for heavy data entry.
- Travel: Valid driver’s license and reliable vehicle for local client assessments.
