Wage Rate: $33.90 – $37.67 per hour

Sunbeam Community & Developmental Services with over 500 employees is a large non-profit, multi-division, multi-site, organization that is committed to serving individuals living with complex developmental, intellectual, behavioural, and/or physical disabilities.

Developmental Services Ontario Central West Region, administered by Sunbeam Community and Developmental Services, is the contact point for adult developmental services in Dufferin, Halton, Peel, Waterloo, and Wellington.

The Intake Transition Coordinator is the point of first contact for MCCSS adult developmental services and supports in the Central West Region with a focus on individuals 16 to 21 years. Responsible for confirmation of eligibility, referral for application package, and processing and maintaining applicant records for adults who have a developmental disability and/or dual diagnosis who are seeking MCCSS funded developmental services.

Specific job duties include, but are not limited to, the following:

Provide information to individuals and the public about how to apply for MCCSS funded adult developmental services and supports, including the application process
Responsible for providing resources and presentations to school boards, individuals, and families, and offering consultation to other professionals regarding MCCSS services and supports for the purpose of transition planning and referral.
Obtain and review applicants information to confirm eligibility as per MCCSS requirements, directing as needed to community resources to obtain documentation
Communicate eligibility outcome to applicants/families by letter
During eligibility process if applicant identifies urgent need, complete referrals to service coordination and/or service solutions
Actively participate on internal and external committees as required
Maintain all necessary documentation and records in DSCIS and EMHWare

The successful applicant will have:

A university degree or college diploma in a related social services field (Sociology, Psychology, Autism, Developmental Services, Community Services) combined with experience serving individuals of all ages with a developmental disability in a variety of community settings and programs
Demonstrated understanding and respect for people of diverse backgrounds
Excellent verbal and written communication skills
Strong consulting skills
Proficiency with MS Office and other computer-related equipment and programs
Ability to work independently and as part of a multi-disciplinary team
Possession of a valid vehicle operator’s license and clean driving record.
Certification in CPI-Prepare Training would be an asset.
Require some flexibility, including evenings and occasional weekends.
