Full job description

Job Overview
We are seeking a compassionate and dedicated Social Worker or Psychotherapist to join our team. The ideal candidate will work in a medical clinic setting, providing support and resources to patients/clients and their families. This role involves assessing patient/clients needs, developing treatment plans, and coordinating services to ensure optimal patient/client outcomes. The Social Worker/Psychotherapist will play a crucial role in advocating for patients/clients, particularly children, and navigating the complexities of healthcare systems.

Duties

Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
Develop and implement individualized treatment plans in collaboration with healthcare teams.
Provide case management services to ensure continuity of care for patients.
Facilitate communication between patients, families, and medical staff to promote understanding of treatment options.
Offer counselling and support to patients/clients and their families during difficult times.
Maintain accurate records of patient interactions and interventions in compliance with The Couch Counselling policies.

Skills

Strong research skills to identify resources and best practices in social work.
Experience in case management.
Ability to provide compassionate patient/client care while maintaining professional boundaries.
Proficiency in assessment methods for evaluating patient progress and program effectiveness.
Excellent communication skills for effective collaboration with interdisciplinary teams.
Empathy and patience when working with children and their families facing challenging situations.
