About FRAFCA:

The Fraser Region Aboriginal Friendship Centre Association is a non-profit organization that provides a diverse range of culturally safe and supportive services to Indigenous People residing in the Fraser Region. As a Friendship Center, we work to strengthen connections to culture, community and wellness and work from a decolonizing perspective. Our philosophy is to provide a culturally safe, collaborative wrap around model of care.

About Surrey:

The City of Surrey is one of 21 municipalities in Metro Vancouver in British Columbia, Canada. We are located on the Lower Mainland on the south side of the Fraser River and share a border with the United States of America. Surrey is situated on the traditional, ancestral, and unceded territories of the Salish Peoples, including

Job Summary:

Reporting to the Housing Program Manager, the Aboriginal Homeless Prevention Program (AHPP) worker supports individuals from identified at-risk groups who are facing or at risk of homelessness. The program provides portable rent supplements and housing support services to help clients secure and maintain housing in the private, non-subsidized rental market. As an AHPP worker, you will assist clients in navigating housing options, accessing community-based services, and building long-term housing stability. You will play a key role in managing and administering rental supplements, ensuring timely payments, tracking subsidy usage, and coordinating with landlords and housing providers. In addition to housing-focused support, you will also assist clients in accessing community-based services and navigating housing-related challenges.

The AHPP worker often works in collaboration with the Aboriginal Homeless Outreach Program (AHOP), with focus on individuals at transition points such as those exiting institutional care, fleeing violence, or aging out of care, who are especially vulnerable to homelessness

Required Qualifications, Skills, and Abilities:

Previous front-line support work experience, or someone with a lived-experience, or a self-starter who can demonstrate commitment to ensuring successful implementation of the program.
Social Service Worker certificate and/or related certificates in human service field; diploma preferred.
Demonstrated understanding of homelessness in Surrey and the lower mainland, including direct service experience and insight into the diverse needs and lived realities of affected communities.
Demonstrated ability to work collaboratively in a culturally safe and respectful manner, contributing to the well-being of the agency and the housing & homelessness team.
Proficient in computer operations, including Microsoft Word, Excel, and Outlook, with a demonstrated ability to quickly learn and adapt to new programs used in case management and client support.
Able to meet the physical and mental demands of the role while bringing a calm, compassionate, and supportive presence to those being served.
Strong oral and written communication skills, with the ability to engage respectfully and effectively with individuals from diverse backgrounds.
Demonstrated reliability through consistent attendance and punctuality
Must have access to a reliable vehicle, a valid Class 5 driver’s license, and a clean driver’s abstract, as the role may occasionally involve supporting clients through wellness checks or house viewings.
All positions are subject to a criminal record check (CRC) & vulnerable sector check (VSC) review.

Job Duties & Responsibilities:

The Homeless Prevention Program Outreach Worker provides the following services:

Collaborate with outreach teams and partner agencies to ensure coordinated and culturally appropriate support for individuals at risk of homeless, particularly during key transition period.

Conduct client intakes and assessments in a culturally sensitive and trauma-informed manner to determine eligibility for the Homeless Prevention Program (HPP) and identify immediate housing and support needs.

Administer and manager rent supplements by coordinating payments maintaining accurate records, and liaising with landlords and/or housing providers to support stable housing placements.

Support clients in securing and maintaining private market housing, including assistance with housing searches, applications, and tenancy-related issues, while respecting diverse cultural background and lives experiences.

Develop individualized service plans in collaboration with clients to identify housing goals and connect them with appropriate community-based supports.

Monitor client progress and provide ongoing case management, including regular check-ins, and client advocacy.

Maintain accurate and timely documentation, including entering data into the Homelessness Individuals and Families Information System (HIFIS), preparing reports, and ensuring compliance with program standards.

Attend internal and external meetings as required to represent FRAFCA, build partnerships, share program updates, and strengthen collaboration with community stakeholders.
