Community Health Nurse (RN/LPN)

Full-Time Position

?Esdilagh First Nation is seeking a compassionate, motivated, and community-focused Community Health Nurse (RN/LPN)to join our team. This position plays a vital role in promoting the health, wellness, and quality of life of community members through culturally safe and holistic healthcare services.

Working collaboratively with community members, health staff, and external partners, the Community Health Nurse will deliver a range of health promotion, prevention, education, and nursing services that support the physical, mental, emotional, and spiritual well-being of the ?Esdilagh First Nation community.

Position Details

Position: Community Health Nurse (RN)
Status: Full-Time
Location: ?Esdilagh First Nation
Salary: Competitive salary based on education, qualifications, and experience
Reports To: Health Director and/or Band Manager

Key Responsibilities

The Community Health Nurse will:

Provide culturally safe and community-based nursing services to individuals and families.
Promote health and wellness through education, prevention, and early intervention programs.
Assess, monitor, and support community members with acute and chronic health conditions.
Coordinate and deliver health promotion initiatives focused on:
Communicable Disease Prevention
Maternal and Child Health
Infant and Early Childhood Development
School Health Services
Chronic Disease Management
Elder Care and Healthy Aging
Environmental Health
Mental Health and Wellness
Immunization Programs
Conduct home visits and community outreach as required.
Support and collaborate with the Community Health Representative (CHR) in delivering health programs and services.
Develop, implement, and evaluate individual care plans.
Coordinate referrals and work collaboratively with healthcare providers, FNHA, hospitals, physicians, and other service agencies.
Provide health assessments, education, and follow-up support to clients and families.
Assist community members in understanding medications, treatment plans, and healthy lifestyle choices.
Participate in emergency response planning and provide nursing support during emergencies.
Maintain accurate client records, reports, and confidential health files in accordance with applicable legislation and professional standards.
Ensure reporting requirements are completed accurately and within required timelines.
Support community workshops, wellness events, and health awareness initiatives.
Participate in program planning, evaluation, and continuous quality improvement activities.
Foster positive working relationships with community members, staff, leadership, and external partners.

QualificationsRequired

Current registration and good standing with the British Columbia College of Nurses and Midwives (BCCNM).
Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
Current CPR Certification.
Minimum one (1) year of nursing experience in community health, public health, emergency, medical/surgical, acute care, or a related healthcare setting.
Valid Class 5 BC Driver's License and reliable transportation.
Strong assessment, communication, and case management skills.
Ability to work independently and as part of a multidisciplinary team.
Strong organizational, documentation, and time-management skills.
Ability to maintain confidentiality and exercise sound professional judgment.

Assets

Experience working with Indigenous communities.
Knowledge of First Nations health programs and services.
Experience with health promotion and community wellness programming.
Understanding of culturally safe and trauma-informed care practices.

Conditions of Employment

Successful Criminal Record Check (Vulnerable Sector Check).
Valid First Aid and CPR Certification.
Ability to travel as required for training, meetings, and community health initiatives.
