Cedars is located in Cobble Hill, British Columbia and situated on 60 acres of forested, private land, which provides a picturesque landscape and tranquil therapeutic setting. Vancouver Island is renowned for spectacular West Coast scenery and features a rich network of recovery communities.
Cedars' Continuing Care program provides one year of structured post-treatment support, including one-on-one recovery coaching, facilitated recovery-support groups, alumni programming, family services, and connections to community resources.
POSITION SUMMARY :
Reporting to the Manager of Continuing Care, the Recovery Coach supports individuals as they prepare to leave residential treatment and throughout their first year after discharge. Using a strengths-based, patient-centered, non-judgmental coaching approach, the Recovery Coach helps participants identify goals, strengthen recovery capital, overcome practical barriers, connect with community resources, and sustain engagement with recovery supports.
The role combines relationship-based coaching, structured outreach, group facilitation, service coordination, documentation, and team communication. The Recovery Coach works within an approved recovery-coaching scope and promptly escalates clinical, medical, safety, legal, or ethical concerns to the appropriate professional.
DUTIES AND RESPONSIBILITIES:
The duties below are representative and are not exhaustive. Other duties must be reasonably related to the purpose, level, and scope of this position.
Transition and In-Patient Engagement:
- Build rapport with patients during residential treatment and explain the purpose, boundaries, and available services of Continuing Care.
- Complete a warm handoff from the primary treatment team and help each participant identify practical, patient-led recovery goals for the transition home.
- Help participants access approved digital tools, virtual groups, Cedars Trails app, alumni supports, supportive recovery housing information, and relevant community resources.
- Document contacts and promptly communicate material concerns, barriers, and follow-up needs to the appropriate team member.
Recovery Coaching and Continuing Care Outreach:
- Complete scheduled outreach, coaching contacts, reviews, and milestone acknowledgements throughout the twelve-month Continuing Care period.
- Use motivational interviewing, active listening, open and evocative questions, goal-setting, and reflective practice to help participants identify their own strengths, choices, and next steps.
- Support participants to build recovery capital across personal, social, community, cultural, spiritual, housing, health, education, and employment domains, as appropriate to their goals.
- Help participants anticipate barriers, strengthen plans for managing recurrence risk, and reconnect with supports following disengagement or a return to use.
- Provide accurate internal and external referrals within role scope and follow through on agreed connections when appropriate.
- Prepare participants for transition to Alumni Services as they approach the completion of the one-year program.
Group Facilitation and Alumni Engagement:
- Facilitate approved in-person and virtual recovery-support or process groups using established curriculum, group agreements, and safety expectations.
- Create an inclusive, trauma-informed, culturally safe environment that respects multiple pathways of recovery while remaining consistent with Cedars’ programming.
- Track attendance, document relevant contacts, and promptly escalate participant concerns or risks.
- Support Cedars alumni events, milestone activities, and community engagement as assigned; occasional local travel or schedule flexibility may be required.
Documentation, Coordination, and Team Practice:
- Maintain accurate, timely, objective, and confidential records in CIMS and approved tracking systems.
- Maintain assigned caseload, contact schedules, active-participant lists, attendance records, referrals, and follow-up tasks.
- Participate in Continuing Care case conferences, supervision, team meetings, training, and quality-improvement activities.
- Collaborate with Continuing Care, Clinical, Medical, Admissions, Support, Alumni, Housing, and other teams to support continuity of care.
- Recognize and promptly escalate clinical deterioration, safety concerns, suspected abuse, imminent risk, medical concerns, privacy issues, boundary concerns, or other matters outside coaching scope.
- Maintain professional boundaries, confidentiality, respectful communication, and appropriate use of lived experience in every interaction.
QUALIFICATIONS:
REQUIRED:
- Completion of an approved foundational recovery-coach training program, or willingness and ability to complete approved training within the onboarding period established by Cedars.
- Relevant experience gained through employment, volunteer service, peer support, recovery-community involvement, community services, continuing care, mental health and addictions, or an equivalent combination of education and experience.
- Demonstrated ability to build trust while maintaining confidentiality, professional boundaries, and a non-judgmental, patient-centered approach.
- Strong verbal communication, active-listening, group-facilitation, documentation, organization, and follow-through skills.
- Ability to use Microsoft 365, video-conferencing tools, electronic records, and tracking systems.
- Ability to work independently within role scope and collaboratively within an interdisciplinary team.
- Current First Aid and CPR certification, or ability to obtain it within the required onboarding period; willingness to complete Cedars-required naloxone, emergency-response, privacy, cultural-safety, and workplace-safety training.
- Satisfactory Criminal Record Check, including the Vulnerable Sector.
- Valid Class 5 BC driver’s license and satisfactory driver’s abstract when travel or patient transportation forms part of the assignment.
PREFERRED:
- Canadian Certified Recovery Coach (CCRC), Recovery Coach Professional (RCP), or another recognized recovery-coach credential.
- Post-secondary education in mental health and addictions, social services, counselling, psychology, peer support, or a related field.
- Experience providing structured recovery coaching, continuing-care outreach, recovery-support groups, referrals, and electronic documentation.
- Training in motivational interviewing, recovery capital, recurrence prevention, trauma-informed practice, cultural safety, Indigenous-informed practice, ethics, or professional boundaries.
Lived experience of recovery is valued but is not required. Candidates who voluntarily identify as being in recovery must have at least one year of continuous substance-free time and be actively engaged in a stable program of recovery.
Successful applicants will display a keen interest in recovery-oriented care and be able to model a healthy and balanced lifestyle.
WORKING CONDITIONS:
- Full-time, primarily on-site work in Cobble Hill, with telephone and virtual coaching contacts and occasional local travel.
- Schedule flexibility may occasionally be required for groups, alumni events, participant needs, meetings, or operational coverage.
- Regular interaction with people experiencing stress, uncertainty, disengagement, recurrence risk, or complex recovery needs.
- Frequent computer, telephone, documentation, meeting, and group-facilitation work, along with movement across the Cedars campus.
