Overview
Reporting to the Director of Programs, the Case Manager – Community Connections & Empowerment Initiative (CCEI) is responsible for delivering Accessible Housing's Community Connections & Empowerment Initiative to enhance the dignity, independence, and quality of life for clients across Accessible Housing's Inclusio, Chinook, and Bridge to Home programs.
Using a strengths-based, trauma-informed, and person-centered approach, the CCEI Case Manager supports participants in achieving goals related to community integration, wellness, employment readiness, and social inclusion. Through assessments, service coordination, advocacy, community engagement, and employment support, the position helps participants build meaningful connections, increase independence, reduce isolation, and actively participate in community life.
The CCEI Case Manager works collaboratively with Program Case Managers and community partners to coordinate participant supports and promote positive outcomes related to wellness, belonging, employment, and community participation
This position requires strong relationship-building skills, a high level of flexibility, excellent organization and communication abilities, and an understanding of the barriers faced by individuals with disabilities, mental health concerns, social isolation, and employment challenges.

Participant Assessment and Planning
- In collaboration with participants and Program Case Managers, complete assessments, develop and implement individualized Community Integration Plans that support participant goals related to wellness, employment, community participation, and independence.
- Identify participant strengths, interests, goals, barriers, and support needs.
- Establish measurable goals and regularly review participant progress.
- Ensure Community Integration Plans complement existing participant service plans and support coordinated service delivery.
Case Management and Service Coordination
- Coordinate services across all components of the Community Connections & Empowerment Initiative.
- Collaborate with Program Case Managers to ensure coordinated participant support.
- In collaboration with Program Case Managers, support participants in navigating health, social, educational, employment, and community resources.
- Advocate on behalf of participants to reduce barriers to participation and inclusion, while promoting self-advocacy and self-determination.
- Provide ongoing case management support using a strengths-based, trauma-informed, and person-centered approach.
- Assist participants in identifying and overcoming barriers to community participation, employment, volunteer opportunities, and personal growth.
- Support participants in accessing community resources, health services, educational opportunities, recreation programs, employment supports, and other community-based services.
Mental Health and Wellness Service Coordination
- Coordinate and facilitate participant access to mental health and wellness services delivered by partner organizations.
- Support participant engagement in counselling, psychoeducational workshops, peer support groups, and wellness programming.
- Encourage the development of strategies that promote resilience, emotional wellness, and community participation.
- Monitor participant engagement and identify barriers to participation.
Physical Wellness Service Coordination
- Coordinate and facilitate participant access to physiotherapy and wellness services delivered by partner organizations.
- Support participants in developing and maintaining personal wellness goals.
- Encourage participation in physical activity, recreation, and community-based opportunities.
Employment Readiness and Workforce Participation
- Support participants in exploring employment, volunteer, educational, and training opportunities
- Facilitate employment readiness activities including resume development, interview preparation, job search support, and workplace readiness.
- Facilitate referrals to employment service providers and workforce development programs
- Build and maintain relationships with employers, workforce development organizations, and community partners.
- Support job placement, onboarding, workplace accommodations, and retention efforts.
- Assist participants in understanding employment opportunities within evolving disability support systems, including ADAP-related workforce participation expectations.
Community Connection and Group Programming
- In collaboration with Program Case Mangers, facilitate participation in volunteer, recreational, educational, cultural, and community-based opportunities.
- Promote opportunities for participants to contribute their talents, skills, and lived experiences within their communities.
- Support activities that reduce social isolation and strengthen social networks.
Partnership Development and Collaboration
- Develop and maintain effective working relationships with community agencies, employers, healthcare providers, educational institutions, and service organizations.
- Serve as the primary liaison for project partners delivering mental health and physical wellness, and employment services.
- Participate in community networks and collaborative initiatives that enhance participant outcomes.
- Promote awareness of the Community Connections & Empowerment Initiative and advocate for inclusive community opportunities.
- Identify opportunities to strengthen partnerships and expand participant access to supports and services.
Outcome Measurement & Reporting
- Track participant engagement, progress, barriers, and outcomes in accordance with program standards.
- Collect and track participant outcome data related to mental wellness, community participation, social connection and workforce participation.
- Provide outcome data and reporting to support program evaluation, funder reporting, and continuous improvement activities.
- Prepare reports, summaries, and required documentation within established timelines.
- Ensure all documentation is complete, accurate, and maintained in accordance with organizational and funder requirements.
- Contribute to quality improvement initiatives through accurate data collection, analysis, and reporting.
Administration and Professional Practice
- Participate in team meetings, case conferences, agency meetings, and staff development activities as required.
- Notify the Director of Programs of any situation presenting actual or potential risk, liability, or safety concerns.
- Maintain professional boundaries and uphold the highest standards of confidentiality, ethics, and professional conduct.
- Ensure compliance with organizational policies, procedures, funder requirements, accreditation standards, and applicable legislation.
- Complete other duties as assigned.
Qualifications
- Degree or diploma in Social Work, Human Services, Disability Studies, Community Rehabilitation, Career Development, or a related field.
- Minimum three years of experience providing case management, community support, employment support, disability services or related human services.
- Experience supporting individuals with disabilities, mental health concerns, homelessness, or complex barriers is considered an asset.
- Experience coordinating services across multiple systems and community partners.
- Experience facilitating groups, workshops, or community-based programming.
- Knowledge of community resources, employment supports, and disability-related services.
- Strong understanding of disability inclusion, community integration, recovery-oriented practice, and social determinants of health.
- Experience supporting employment readiness and workforce participation is considered a strong asset.
- Strong interpersonal and relationship-building skills.
- Excellent organizational and time management skills.
- Proficiency in Microsoft Office applications including Word, Outlook, Excel, and Teams.
- Ability to work independently with limited supervision.
- Strong administrative, communication, and documentation skills.
- Proficiency in data collection, documentation, and outcome measurement.

Working Conditions
- The CCEI Case Manager works in a residential program setting, office environments, and community locations.
- Regular travel between Accessible Housing program locations and community partner sites is required.
- The position works a regular schedule with flexibility to accommodate occasional evenings and weekends to support participant engagement and community activities.
- The CCEI Case Manager works with participants experiencing complex barriers, including disabilities, mental health concerns, social isolation, unemployment, and housing instability.
- The position may be required to respond to participant crises, urgent support needs, and changing service priorities.
- The CCEI Case Manager works collaboratively with Program Case Managers across Inclusio, Chinook, and Bridge to Home to support coordinated service delivery and participant outcomes.
- The CCEI Case Manager provides backfill coverage for the Case Manager, Inclusio & Chinook during vacations and approved absences.
- The CCEI Case Manager is required to follow all organizational safety policies, procedures, and risk management practices to ensure the safety of participants, staff, and community partners.
