Case Manager, Bridge to Home

Full-Time | Calgary, AB

Help people move from homelessness to home.

Accessible Housing is looking for a compassionate, skilled Case Manager to join our Bridge to Home team. If you're passionate about Housing First, believe everyone deserves safe, accessible housing, and thrive in community-based work supporting people with complex needs, we'd love to hear from you.

Bridge to Home provides intensive, individualized support to people living with disabilities who are experiencing homelessness or housing instability. As a Case Manager, you'll walk alongside participants as they navigate housing, health, income, community connections, and recovery—helping them build stability and achieve their own goals.

This is a role for someone who enjoys meaningful relationship-building, creative problem solving, and working collaboratively to remove barriers and create lasting change.

What You'll Do

Working within a scattered-site Housing First and Recovery in Housing model, you will:

Provide intensive case management to a caseload of participants with diverse and complex needs.
Complete assessments, develop individualized service plans, and support participants in achieving their goals.
Build trusting relationships through consistent outreach, engagement, advocacy, and follow-up.
Support participants in accessing housing, income supports, healthcare, mental health and addiction services, identification, and other community resources.
Navigate complex systems while advocating for participants and empowering them to build self-advocacy skills.
Respond to crises and provide stabilization support using trauma-informed, person-centered, and strengths-based practices.
Collaborate with landlords, healthcare providers, community agencies, and other service partners to coordinate wrap-around supports.
Maintain accurate documentation and outcome reporting using HMIS and internal systems.
Contribute to a collaborative team committed to continuous learning and quality service delivery.

We're Looking For Someone Who Has

A Diploma or Degree in Social Work, Human Services, Community Rehabilitation, Psychology, Disability Studies, Addictions Counselling, or a related field.
At least two years of experience providing case management, outreach, housing support, or similar services to individuals experiencing complex barriers.
Experience working within the homelessness-serving sector, Housing First programs, disability services, mental health, addictions, or supportive housing is a strong asset.
Strong understanding of trauma-informed practice, harm reduction, recovery-oriented approaches, and person-centered support.
Excellent relationship-building, communication, and organizational skills.
The ability to work independently while managing multiple competing priorities.
Confidence navigating community resources and advocating within complex service systems.
Proficiency with Microsoft Office and electronic documentation systems.
A valid driver's licence, reliable vehicle, and a satisfactory Criminal Record Check with Vulnerable Sector Search.

About the Role

This is primarily a community-based position. You'll spend much of your time meeting participants where they are—in their homes, shelters, healthcare settings, community agencies, and throughout Calgary. Every day is different, requiring flexibility, sound judgment, and the ability to adapt to changing priorities.
