Position Summary: The Lead, Health & Wellness provides leadership for the Fairview Suites Retirement Home, ensuring residents receive safe, high-quality, person-centred care and services. This role oversees the day-to-day coordination of care, leads and supports the nursing, personal support, and housekeeping teams, and is responsible for quality improvement, medication management, staffing coordination, regulatory compliance, and resident well-being. Working collaboratively with residents, families, and the interdisciplinary team, the Lead, Health & Wellness fosters a culture of excellence, accountability, and compassionate care while supporting a positive team environment and the ongoing success of the Retirement Home.

Reports to: Director, Retirement & Community Care

Assets and Qualifications

Current registration as a Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario is an asset.
Minimum two (2) years of experience in retirement living, long-term care, geriatrics, or a related healthcare setting.
Demonstrated knowledge of retirement home legislation, best practices, medication management, and resident-centred care.
Strong leadership, communication, organizational, and problem-solving skills.
Experience with quality improvement, regulatory compliance, and performance management.
Ability to work collaboratively with physicians, pharmacies, residents, families, and interdisciplinary teams.
Proficiency with electronic health records and Microsoft Office applications.

Position Specific Responsibilities include, but are not limited to:

Provide clinical and operational leadership for the Retirement community, ensuring the delivery of safe, high-quality, resident-centred care.
Promote and support Fairview Community's mission, values, and faith-based commitment while fostering a culture of kindness and person-centred care.
Participate in the assessment of prospective residents to determine the appropriateness of placement.
Oversee the coordination and maintenance of resident health records to support quality care and regulatory compliance.
Supervise the medication management program, including the administration of medications by unregulated care providers in accordance with legislation and organizational policies.
Collaborate with residents, families, and the interdisciplinary team to coordinate care and support resident well-being.
Work collaboratively with the Resource Coordinator to ensure appropriate staffing levels and effective scheduling of nursing, personal support, and housekeeping teams.
Provide day-to-day leadership, coaching, and supervision to team members, including orientation, probationary reviews, annual performance evaluations, and ongoing performance management.
Coordinate and support ongoing staff education, mandatory training, competency development, and clinical learning opportunities.
Facilitate team meetings, communicate organizational initiatives, and foster a positive, collaborative team culture.
Participate in the development, implementation, review, and continuous improvement of clinical policies, procedures, and standards.
Lead quality improvement initiatives, audits, investigations, and action plans to support continuous improvement in collaboration with the Director, Retirement & Community Care.
Maintain current knowledge of evidence-informed best practices in seniors' care and serve as a clinical resource for the team.
Respond to resident, family, and team member concerns in a timely and professional manner, supporting effective resolution.
Participate in resident care conferences and interdisciplinary care planning as required.
Ensure adequate supplies, equipment, and resources are available and properly maintained.
Comply with all organizational policies, infection prevention and control practices, WHMIS, occupational health and safety requirements, and emergency preparedness procedures.

Shift Information:

Primarily Monday - Friday but may include evening, weekend or work on holidays.

Participate in the on-call rotation, as required.

Full-time, Permanent

Salary range: $64,724 - $77,179
