Full job description

Position Overview

The Chatham Kent Family Health Team (CKFHT) provides primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in the Chatham-Kent region. The FHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care.

The Pharmacist will play an integral role in the care of the patients of the

The Chatham Kent Family Health Team. The Pharmacist will assist the FHT in achieving its

vision and mission. The Pharmacist will support patient care and the delivery of programs

including chronic disease, palliative care, cancer care, mental health, and geriatric care.

The pharmacist will also provide physicians and Team providers with pharmacological

consultation, education and information that contributes to the health and wellness of

patients and the knowledge and expertise of providers. This position is eligible for HOOPP.

Roles and Responsibilities

· Accept patient referrals from FHT physicians and interdisciplinary providers, following established protocols to identify and resolve medication-related issues and contribute to treatment and care plans.

· Collaborate with primary care providers to develop, monitor, and follow up on pharmacotherapeutic treatment plans.

· Document all patient care activities in the electronic medical record and share results of assessments and interventions with referring physicians and team members within established timeframes.

· Optimize medication management for chronic conditions such as hypertension, diabetes, dyslipidemia, and heart failure in collaboration with family physicians.

· Participate in the development and delivery of targeted programs, including palliative care, cancer care, chronic disease management, mental health, and addictions.

· Apply self-management strategies to support and engaged patients while developing and maintaining educational resources for both patients and providers.

· Provide current, evidence-based information to physicians and team members regarding new medications, contraindications, and therapeutic guidelines.

· Lead individual and group education sessions for patients and providers and evaluate patient care outcomes in collaboration with the team.

· Facilitate referrals and ensure smooth transitions for medication-related care between primary care, hospitals, and home care services.

· Assist in planning, implementing, and evaluating chronic disease management and medication-related health programs, including anticoagulation therapy.

· Establish and maintain policies for medication management, ensuring consistency in handling discontinued medications, safety updates, and contraindications.

· Recommend improvements to medication use processes, enhancing safety and efficiency in sample handling, administration, and documentation.

· Collect and analyze data for reporting purposes and provide timely updates as requested by the Team Lead and/or Executive Director.

· Observe, collect, and record patient data in the EMR while contributing to team building, program development, and process improvement initiatives.

· Participate in staff meetings to exchange ideas, support colleagues, monitor program outcomes, and present case studies.

· Assist in developing and implementing collaborative care guidelines, ensuring compliance with personnel policies and procedures.

· Identify service gaps and recommend modifications as needed.

· Facilitate communication and networking between primary care, hospital, and home care services while maintaining professional relationships with relevant networks and organizations.

· Represent the FHT’s mission and values in community meetings, presentations, and advocacy efforts.

· Support a collaborative and inclusive work environment for staff and patients, helping to train and supervise students and volunteers as needed.

· Assist patients in identifying and utilizing health resources while empowering them to take an active role in their care through self-management and education initiatives.

· Support health promotion activities, identify community needs and resources, and provide clinical leadership.

· Participate in performance evaluations and professional development activities, setting goals for continued growth.

· Stay informed through relevant training, workshops, and conferences.

· Attend community meetings as required and perform additional duties as assigned by the Team Lead and/or Executive Director.

Qualifications and Requirements:

· B.Sc (Pharmacy) and registration in Part A of the Ontario College of Pharmacists. Community or Hospital Pharmacy residency or equivalent clinical experience in either setting is required

· Additional training or credentialing in pharmacotherapy is considered an asset (i.e.. Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma Educator)

· Demonstrated clinical skills in assessment, program planning, implementation and evaluation.

· Minimum one year of demonstrated clinical experience

· Work effectively, both independently and as part of an inter-disciplinary team.

· Proven flexibility and ability to adapt to change.

· Computer skills.

Job Type: Part-time

Pay: $50.00-$56.00 per hour
