The Personal Support Worker (PSW) is a valued member of the home care team who works in partnership with a Care Designer and Care Team to provide personal care and daily living support while assisting clients aging safely, comfortability, and happily in their own homes.
Through trusted relationships, thoughtful attention to individual needs, and dependable care, the PSW plays a meaningful role in turning care plans into positive daily experiences that support Happier Aging.
Immediate Supervisor: Care Designer
Essential Duties and Responsibilities:
The Personal Support Worker delivers care with professionalism, accountability, and Bold Kindness, including but not limited to:
- Provides client care in alignment with the established plan of care, with clinical guidance and support from the Director of Nursing or Registered Nurse as required.
- Assists with personal care and hygiene in a respectful, person-centered manner, including bathing, grooming (oral care), dressing, toileting, and continence care, meal support, feeding, etc.
- Supports skin integrity and mobility by repositioning, encouraging movement, and following skin care protocols.
- Assist with active and passive range-of-motion exercises as per care plan
- Supports safe ambulation, transfers, and use of mobility aids
- Provides medication reminders to support client self-administration, as permitted by scope.
- Observes and communicates changes in the client’s physical, mental, or emotional condition to the appropriate Care Designer immediately.
- Maintain a proper record of care provided and observed changes accurately and promptly.
- Performs light housekeeping to maintain a clean, safe, and comfortable home environment, including changing bed linens, dusting, vacuuming, etc.
- Assists with meal planning and preparation in a clean and hygienic environment in alignment with client preferences and care plans.
- Uses equipment and supplies safely and appropriately.
- Follows principles of infection prevention, control, and universal precautions at all times.
- Assists in maintaining a safe environment and promotes quality care through adherence to established policies, procedures, and standards, working collaboratively with the care team.
- Provides transportation support to medical appointments, community outings, and shopping as required.
- Participates in performance improvement activities and team meetings, recognizing that high-quality care is built through learning and consistency.
- Participates in client education to reinforce safe practices and independence.
- Upholds client rights (privacy, respect, choice, confidentiality) at all times.
- Responds appropriately and promptly in emergencies.
- Record vital signs (temperature, pulse, respiration rate, and blood pressure) as per the care plan. Additionally, measure and document the weight, height, and/or intake and output as required.
Qualifications:
- Required: Certification as a Personal Support Worker or Home Care Aide from a recognized community college or vocational school.
- Required: Current CPR certification
- Negative TB skin test or chest x-ray, as required.
- Criminal check with vulnerable sector
- 2 Professional References
- Minimum of one (1) year of relevant experience preferred.
- Effective oral and written communication skills in English
- Valid driver’s license and access to an insured vehicle in accordance with organizational requirements.
- Ability to work both collaboratively as part of a team, and independently with minimal direct supervision, while maintaining contact with the local care team.
- Demonstrated empathy, reliability, and respect for the needs of clients and families.
- Commitment to treating clients, team members, and the public with courtesy, respect, and professionalism while maintaining confidentiality and security.
Job Limitations:
The Personal Support Worker (Home Care) does not perform duties considered the practice of nursing under the Regulated Health Professions Act. Specifically, the Personal Support Worker does not administer medications, take physician orders, or perform procedures that require the training, knowledge, and skills of a registered nurse.
Working Conditions & Physical Demands:
- Regular contact with clients in a variety of settings (primarily clients’ homes; occasionally in institutions/hospitals)
- Travel is required.
- Potential exposure to blood and other potentially infectious materials (OPIM).
- Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull as required to support client care.
- Ability to lift up to 50 pounds, with assistance as appropriate.
- PPE: Wear gloves, masks, and eye/face protection as required to perform care safely
