Program Associate, WeConnect

Temporary Full-Time – 9-Months with the possibility of extension

At Community Care Durham (CCD), coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people wherever they choose to call home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Supervisor, Mental Health and Wellness, the Program Associate, WeConnect, provides support for various areas of the WeConnect Mental Health program, including client services, volunteer support, and statistical reporting. The Program Associate is also responsible for conducting follow-up with clients to assess ongoing service needs, as well as responding to program inquiries and client requests in a timely and professional manner.

Responsibilities include but not limited to:

Contacts clients to provide follow-up as requested by group facilitators (i.e., attendance inquiries, simple referral, etc.) or as initiated by clients;
Communicates with clients to determine interest in continuing WeConnect services;
Enters client’s information into database and ensures accurate client information;
Inputs and monitors weekly group attendance to identify when client follow-up is required;
Manages shared email inboxes and responds to inquiries in a professional and timely manner;
Uploads and manages all group notes submitted by volunteer facilitators;
Maintains and updates active WeConnect volunteer list;
Provides administrative and coordination support to the Volunteer Coordinator, as assigned;
Perform other duties as required.

Qualifications required:

Post secondary education in Mental Health, Social Services, or related field
Experience working with individuals from diverse backgrounds experiencing a variety of mental health concerns is an asset
Strong organizational skills and attention to detail
Strong verbal and written communication skills
Ability to work well under pressure to meet deadlines through flexibility and multitasking
Experience in community-based mental health services an asset
Sound knowledge of customer service practices
Comprehensive knowledge of Microsoft Office Programs (Outlook, Word, PowerPoint, Excel) and virtual meeting space platforms (Zoom, Microsoft Teams)
Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition  
Clear Vulnerable Sector Police Check completed within the past 6 months. (Please note: 4–6 weeks processing time)
Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Hourly base rate of $21.00; with an additional 5% in lieu of benefits, and 6% vacation pay

Work Location: Whitby, ON (Hybrid)

*Subject to change, CCD staff may be required to work anywhere throughout the Durham Region

Hours of Work: 8:30am – 4:30pm, Monday to Friday (35 Hours/Per Week)
