Key Responsibilities

Provide comprehensive nursing care to patients in community and home care settings.
Conduct patient assessments and develop, implement, and evaluate individualized care plans.
Administer medications, treatments, and wound care as prescribed.
Monitor patient health status and communicate changes to physicians and members of the healthcare team.
Educate patients, families, and caregivers on health management, treatment plans, and self-care techniques.
Promote patient independence, safety, and well-being while supporting care in the home environment.
Maintain accurate and timely clinical documentation in accordance with professional and organizational standards.
Coordinate care and collaborate with interdisciplinary teams to ensure seamless service delivery.
Travel between patient homes and community locations as required to provide care.
Adhere to infection prevention and control practices, workplace safety standards, and nursing best practices.
Build trusting relationships with patients, families, and community partners to support positive health outcomes.
Respond effectively to changing patient needs and provide compassionate, patient-centered care in diverse community settings.

Job Qualifications

Our “exemplary standing” with Accreditation Canada held to the highest regard and is a testament to the fact that we uphold the same hiring standards and patient care delivery as the facilities in which we serve.

Graduate of an accredited Registered Nurse (RN) program.
Current and unrestricted registration in good standing with the applicable provincial nursing regulatory body.
Current CPR/BLS certification.
Minimum one (1) year of recent nursing experience in community health, home care, acute care, primary care, or a related clinical setting preferred.
Strong clinical assessment, critical thinking, and care planning skills.
Experience with wound care, chronic disease management, medication administration, and patient education is considered an asset.
Excellent communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare partners.
Ability to work independently and make sound clinical decisions in community-based settings.
Proficiency with electronic documentation and mobile technology.
Valid driver's license and access to a reliable vehicle for travel between patient visits.
Willingness to travel and work in rural and remote communities, including Manitoulin Island.
Demonstrated commitment to providing compassionate, culturally sensitive, and patient-centered care.
Flexibility to work a variety of shifts and assignment lengths as required.

If successful, new hires will be required to provide proof of vaccination prior to their start date.
