Health for All Family Health Team is an inter-professional primary care clinic. We provide a wide range of primary care services including acute, chronic and preventative health care. Our physicians and other health professionals have diverse interests such as palliative care, geriatrics, home visits, low risk obstetrics, and global health.
A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for our patients.
Health for All is also the site of the Markham Family Medicine Teaching Unit (FMTU). The FMTU is a partnership between Oak Valley Health- Markham Stouffville Hospital and the University of Toronto Department of Family and Community Medicine and Health for All.
About the Opportunity:
The Health for All Family Health Team (HFAHT) is seeking a passionate and skilled Clinical Pharmacist to join our growing interprofessional primary care team. We are committed to delivering comprehensive, team-based care that improves health outcomes for our diverse community.
As a Clinical Pharmacist at HFAHT, you will be embedded within family physician practices — serving as a medication expert, patient advocate, and trusted colleague. You’ll lead medication reviews, resolve drug-related problems, support evidence-informed prescribing, and make a tangible difference in patients’ lives every day.
What You’ll Do:
- Conduct individual patient medication assessments and develop personalized care plans in collaboration with physicians and the care team.
- Identify, prevent and resolve drug-related problems — including medication histories, monitoring, and follow-up.
- Prescribe and adjust medications within Ontario College of Pharmacists scope of practice.
- Utilize medical directives for expanded scope activities such as ordering labs and initiating pharmacotherapy.
- Lead academic detailing, education sessions, and quality improvement initiatives within the primary care team.
- Champion health promotion, chronic disease management, and evidence-informed prescribing.
- Mentor learners and primary care colleagues on medication management best practices.
What You Bring:
Required
- BScPhm or PharmD from a recognized university.
- Current registration in good standing with the Ontario College of Pharmacists (Part A).
- Completion of ACPR residency or equivalent clinical experience.
- Demonstrated ability to work in an interprofessional team environment.
Preferred
- 2+ years of experience in primary care.
- Advanced credentials in pharmacotherapy, evidence-based medicine, or pharmaceutical care (e.g., BPS, CDE, TEACH, academic detailing).
- Experience with EMRs: Telus PS Suite, Accuro, Avaros, or OSCAR.
- Group facilitation or program planning experience.
