We are seeking a full-time (1.0 FTE) Registered Nurse to join our team in delivering exceptional care for all clinical programs.
Organizational Overview
The Charles H. Best Diabetes Centre is a one-of-a-kind community healthcare centre and registered charity. Our expertise serves the needs of patients with type 1 diabetes. We care for patients of all ages (toddler to elderly) and work closely with their families and caregivers. We pride ourselves in meeting new patients within hours of diagnosis, providing the care, education, resources and support to safely guide them and their families through the tumultuous early days of this life-changing condition.
We offer intensive clinical programming and services based on a truly integrated and collaborative care model. Our interdisciplinary team includes a Clinical Programs Leader, Registered Nurses, Registered Dietitians, Registered Social Workers, Registered Practical Nurse, and administrative and fundraising teams – all lead by the Executive Director. We are affiliated with many physicians, both pediatric and adult.
Position Overview
As a member of our interdisciplinary educator team, you will work collaboratively with children, youth, adults and their families/caregivers to provide on-going, individualized type 1 diabetes care and guidelines for self-management all supported under The Best Centre’s medical directives. Educators have autonomy in clinical decision making; physician partners are not always on-site or available. The successful candidate will participate in an internal orientation and training program to deliver expert T1D education and care plans for all of our patients.
Hours of operation: Mon/Wed/Fri 9am-5pm; Tues/Thurs 9am-9pm.
The ideal candidate would be available to work five (5) days per week, including one (1) evening.
Location: North Whitby, with opportunities to work remotely one (1) day per week.
Key Responsibilities
· Provide initial and ongoing education, management and support to patients and their families/caregivers, applying best practice standards that are aligned with the College of Nurses of Ontario, the Best Centre’s Clinical Practice Guidelines and the current Diabetes Canada Clinical Standards and Guidelines.
· Provide in-depth assessments to identify and establish priorities for clinical care.
· Work in partnership with patients and families to develop care and education plans that encourage and support self-management, helping to set goals to achieve desired outcomes.
· Develop and deliver education that is tailored to patients’ learning styles and patient/family readiness to move forward in the self-management of their care.
· Monitor and adjust care plans for the substantial proportion of our patients on pump therapy (all types)
· Review patient blood glucose, sensor and pump reports and discuss recommendations.
· Ensure the quality and integrity of all patient information through use of electronic medical records.
· Collaborate with the Executive Director, Clinical Programs Leader, Office Manager and the clinical team in the quality and continuous improvement of the internal programs to meet changing needs of the community.
· Contribute to the achievement of Best Centre goals, as identified by the staff, Executor Director and/or the Board of Directors, and the Strategic Plan.
· Purposefully engage with all staff to build and maintain a respectful, healthy, and balanced work environment.
Qualifications:
- Current RN licence in good standing with the College of Nurses of Ontario is required.
- Type 1 diabetes exposure and chronic illness counselling experience is an asset.
- Acute care and/or pediatric experience is an asset but not required.
- Excellent communication and critical thinking skills as they relate to patients and interdisciplinary collaboration.
- Knowledge of the stages of growth and development throughout the life cycle, the impact that these have on the management of diabetes and on the learning process.
- Demonstrated commitment to ongoing and self-directed professional education and development.
- Comfortable with new and emerging technologies.
