ROLE SUMMARY:
This role combines direct nursing care with on-site supervisory responsibilities, ensuring excellence in both clinical practice and staff support. The individual in this role will participate directly in the client care ratio, providing hands‑on care, comprehensive nursing assessments, ongoing monitoring, and clinical interventions as a key member of the frontline team. Serving as the site supervisor, the Clinical Care Coordinator offers guidance, mentorship, and real‑time support to Safehaven staff. Drawing on their nursing knowledge, skill, and judgment, they will support both staff and clients by delivering in‑depth clinical care recommendations, overseeing the plan of care, and advising on appropriate supportive interventions.
The Clinical Care Coordinator is a key leadership position responsible for overseeing the delivery of high- quality of care, safe and family-centered care for individuals with complex medical needs at Safehaven.
Working within a client‑ and family‑centred model, the Clinical Care Coordinator is accountable for ensuring continuity, consistency, and coordination of care for residential clients. This includes promoting best practices, supporting clinical decision‑making, and helping to ensure that every client receives safe, compassionate, and individualized care.
This individual will report to the Associate Director, Clinical Programs, Quality & Safety, the Client Care Coordinator will work in a partnership with the Clinical Nurse Educator, and the Safehaven leadership team to implement and sustain quality of care and service delivery, such as implementation of key organizational strategies, such as the achievement of Accreditation through Accreditation Canada, and alignment of goals with the Safehaven Strategic Planning.
RESPONSIBILITIES:
- The position follows a Thursday–Monday rotation and includes weekend coverage, with flexibility built in to accommodate operational and individual needs. This role is supported by On-call leadership (including evenings/weekends/holidays).
- Ensures the effective operation of the residence by working collaboratively with the lead Client Care Coordinator to support licensing processes and day-to-day upkeep of the location, contributing to invoicing, security, information technology, site cleanliness, and overall operational needs.
- Supporting staff with client care and/or assessments. Directly supervises employees assessing recruitment needs and maintains the proper staffing complement to ensure correct staffing ratio.
- Supports the completion of employee Performance Appraisals for all staff.
- Verifies schedules and shift assignments.
- Provides assistance with managing sick/vacation time and updating the scheduling system to reflect staffing changes.
- Enters staff schedules and hours worked into the payroll system accurately and on time.
- Ensures that employees receive health & safety training annually, demonstrates a positive “health and safety” attitude, and maintains all health and safety standards and procedures.
- Provides support to front-line staff who care for clients and their families.
- Supports the lead in ensuring that clients’ basic needs are met within a safe, homelike environment:
- Assists in ensuring a safe environment, with appropriate attention to health and nutrition. Helps oversee the upkeep and cleaning of the residence to ensure each client maintains privacy, can retain personal belongings, and remains protected from physical or emotional harm.
- Participates in assigning employee responsibilities as part of a shared leadership approach to developing, monitoring, and maintaining client programs.
- Acts as an advocate on behalf of the clients.
- Supports the lead in ensuring that the location meets all ministry licensing requirements and remains in full compliance with regulatory and Accreditation Canada standards.
- Assists in preparing staff for annual licensing inspections by helping coordinate training, communication, and any required documentation.
- Helps ensure all employee documentation is collected and submitted to Human Resources in a timely and accurate manner in accordance with ministry, regulatory and Accreditation Canada requirements.
- Other duties may be assigned according to the needs of the organization.
QUALIFICATIONS:
- Must be licensed as a Registered Practical Nurse with the College of Nurses of Ontario, and in good standing.
- Desire to work for an organization that is committed to providing exceptional service and be an ambassador of Safehaven values to provide services to clients and families.
- Must possess excellent organizational and time management skills.
- Must have excellent communication skills and be able to communicate with all levels of staff.
- 2-3 years’ related experience with individuals with complex medical needs, or previous experience in a residential setting.
- Ability to work closely with the Safehaven leadership team to support and implement organizational decisions across all areas of operation.
- Excellent interpersonal skills, problem solving and written/verbal communication skills.
- At any time, Safehaven may need to move staff to other locations based on operational needs.
