Full job description

Join Community Living–Central Huron as a Part-Time Supported Independent Living Worker and make a meaningful difference by supporting people to live independently and fully participate in their communities. This role offers the opportunity to build strong relationships, promote choice and inclusion, and support people in achieving their goals

Requirements and Qualifications

Ontario Secondary School Diploma or equivalent and a minimum of 2 – 3 years’ experience in the Human Services field OR Post Secondary School Diploma in a related field and a minimum of 6 months – 1 year experience
Demonstrated ability to independently manage critical and exceptional situations
Strong verbal and written communication skills
Knowledge of generic and specialized services, and relevant provincial and federal legislation
Computer proficiency (Microsoft Office Suite)
Up-to-date certificates in First Aid, C.P.R., Safe Management or C.P.I., and WHMIS
Valid Ontario Driver's License, Class G, with access to and use of a reliable vehicle and Third-Party Liability Insurance in the amount of two million dollars

Key Responsibilities & Skills

Provide individualized, flexible support to individuals living independently, maintaining an assigned caseload and providing on-call or coverage support as needed
Develop and implement person-directed plans and individualized support strategies.
Act as the primary liaison for individuals on caseload, coordinating supports with internal and external partners
Promote independence, inclusion, and quality of life by supporting access to community-based resources and natural supports
Maintain accurate documentation, reports, and records in compliance with Agency policies and Ministry requirements
Ensure all supports align with Agency policies, procedures, and legislative requirements, including health and safety standards
Participate in planning processes, case conferences, and team collaboration to support consistent service delivery
Remain responsive to emergency situations and changing individual needs, including scheduling adjustments

Duties and Core Competencies

Building Relationships: Foster respectful, collaborative relationships with people receiving support, families, community partners, and service providers. Maintain open communication and contribute to a positive, professional, and supportive team environment

Advocacy: Promote individuals’ rights, choices, and self-advocacy while supporting access to services and resources. Provide information on options and natural consequences, and advocate as needed to enhance independence and inclusion

Championing Change and Innovation: Adapt supports to reflect evolving needs, identify and access community resources, and contribute to continuous improvement in service delivery aligned with Agency values and service principles.

Problem Solving and Decision-Making: Independently assess situations and make timely, informed decisions related to people’s needs, often without immediate supervision. Apply sound judgement in both routine and complex situations while ensuring safety and alignment with Agency policies.
