KEY FUNCTION:
Under the supervision of a licensed Pharmacist ensure the safe and effective use of medications within HSN and learn the operational and clinical skills required to be a Pharmacist.
REPORTING:
Under the general direction of the Clinical Manager, Pharmacy.
DUTIES:
1. Provide clinical pharmaceutical care to patients.
2. Provide oral and written follow-up with treatment recommendations to the prescriber as required.
3. Provide medication reconciliation services and interact with patients/families and other health care professionals with regards to patients’ home medication usage.
4. Ensure completion of required documentation in conformance with the standards of practice of the Ontario College of Pharmacists (OCP) and HSN policies.
5. Interact with health care professionals and the patient/family regarding the patient’s plan of care; handle drug-related problems.
6. Ensure verification of all technical work by directly confirming or via to the standards of practice of the OCP and HSN policies.
7. Perform Therapeutic Drug Monitoring (TDM) for high risk drugs.
8. Maintain compliance with departmental policies and procedures.
9. Complete departmental projects as required.
10. Engage in professional self-development.
11. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
12. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
13. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
14. Educate and promote health, safety and wellness in the work place.
15. Represent the department or program on various committees and in meetings as required.
16. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree in Pharmacy, from an accredited university.
2. Doctorate of Pharmacy (PharmD) is preferred.
3. Current Certificate of Registration in good standing with the Ontario College of Pharmacist (OCP) is required.
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Experience working as a Pharmacy Student within a hospital environment is preferred.
2. Experience working in community pharmacy is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of Pharmacy distributive services.
2. Demonstrated knowledge of electronic order entry processes.
3. Demonstrated computer proficiency in clinical systems.
4. Ability to interact with physicians and other healthcare providers in a professional and effective manner.
5. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
6. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
7. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
8. Demonstrated superior interpersonal and communication skills, both written and verbal.
9. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
10. Demonstrated discretion and maturity when handling confidential information.
11. Demonstrated commitment to the safety of co-workers and patients.
