Behavioural Supports Ontario Transitional Lead Position
The Behavioural Supports Ontario (BSO) Transitional Lead (TL) works in collaboration with the client, family, substitute decision maker and community care providers to facilitate a holistic approach to care for persons presenting as high risk for responsive behaviours on transition to a long-term care home (LTCH). Prior to transition, the TL will conduct a full assessment of the long-term care (LTC) eligible client, trial and model strategies to create a Transitional Care Plan to support the client as they move to LTC.
We are currently recruiting for a Regional Full-Time Transitional Lead (RN, Social Worker, or OT) to join our growing and dynamic team!
The successful applicant would work within the Hamilton, Niagara, Haldimand, and Brant regions.
Duties and Responsibilities Overview:
- Demonstrates superior assessment skills including: screening for depression, psychosis, suicide risk, cognitive status, the use of appropriate assessment tools and excellent skills in formulating comprehensive assessments in mental health, treatment and non-pharmacologic therapeutic interventions.
- Develops and maintains an effective working relationship with each LTCH, Ontario Health at Home and external partners.
- Conducts speaking engagements and education in-services in LTCHs in their assigned regional hub regarding their role in transitional support.
- Develops, implements and evaluates quality improvement activities.
- Assesses and prioritizes referrals based on client needs, potential date of admission and care requirements to ensure relationship building can be well established in advance of the LTCH admission.
- Ensures regular and consistent communication through committee involvement and regular reporting as assigned by manager.
- Implements program changes as directed by Ontario Health at Home and authorized by the Director, Community Programs.
- Maintains all required BSO program documentation as per established BSO and regulatory college standards.
- Maintains relationships, ongoing training and awareness with mental health program and supports within the community.
- Attends BSO team meetings.
- Other duties as assigned by the manager.
Skills, Education, and Experience:
- A University degree and current registration in nursing, social work, or occupational therapy is required
- Minimum 5 years demonstrated expertise in working with older adults with mental health illness is required
- Certificates with PIECES, DementiAbility, and GPA are an asset.
- Extensive knowledge, skills and experience in the assessment/care planning for older adults and the use of clinical assessment tools as they relate to complex physical health problems. This may include pain, chronic disease management, delirium, metabolic disorders, cognitive function and impairment including dementia as well as other neurological disorders.
- Experience working in a LTCH or mobile role is an asset.
- Experience in leadership, mentoring or educator roles an asset.
- Superior communication skills (verbal, written and interpersonal) and use of personal-centred language.
- Demonstrates ability to prioritize workload to meet established program requirements. Demonstrates ability to work independently, as well as function within an interdisciplinary team.
- Proficiency in the use of Microsoft 365 and use of electronic medical records.
- Valid G Driver’s licence, insurance and the ability to travel across the HNHB regions/assigned areas is required

- RN License in Good Standing (preferred)
- Social Worker Registration (preferred)
- Occupational Therapy Diploma (preferred)
