Full job description

RAI MDS Coordinator/RPN. Responsible to the Director of Care (DOC), the RAI Coordinator is responsible for coordinating and overseeing the Resident Assessment Instrument (interRAI) program within the Home. This role ensures compliance with legislative requirements, InterRAI standards, and CIHI reporting requirements, and Home policies. The RAI Coordinator promotes accurate resident assessments, person-centred care planning, quality improvement, and data integrity.This is a hybrid position in which the incumbent also provides direct resident care as a Registered Practical Nurse (RPN) during scheduled clinical shifts.

Duties include (but are not limited to):

· Coordinate and oversee the Resident Assessment Instrument (interRAI) process in accordance with applicable legislation, InterRAI standards, CIHI requirements, and Home policies.

· Maintain the resident assessment schedule and ensure assessments, coding, validation, and submissions are completed accurately and within required timelines.

· Monitor Assessment Reference Dates (ARDs), data quality reports, and validation reports, resolving discrepancies as required.

· Ensure resident assessments accurately reflect residents' current clinical status and that care plans are updated following assessments or significant changes in condition.

· Generate and analyze clinical reports, including Quality Indicators (QIs) and Case Mix Index (CMI) data, to support quality improvement, operational planning, and funding.

· Conduct documentation audits and provide education, coaching, and support to interdisciplinary staff on assessment completion, documentation standards, coding, and care planning.

· Coordinate orientation and ongoing education related to interRAI processes and monitor staff competency in accordance with organizational requirements.

· Serve as the Home's clinical resource for resident assessment, documentation standards, and interRAI practices.

· Coordinate resident care conferences and collaborate with the interdisciplinary team to support comprehensive assessments and person-centred care planning.

· Participate in quality improvement initiatives, program evaluations, Ministry inspections, accreditation activities, committees, and audits as required.

· Communicate effectively with residents, families, physicians, nurse practitioners, leadership, and interdisciplinary team members.

· Perform the duties and responsibilities of a Registered Practical Nurse during scheduled clinical shifts in accordance with the College of Nurses of Ontario Standards of Practice and Home policies.

· Perform other duties as assigned by the Director of Care or designate.

· Working knowledge of the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, InterRAI standards, and College of Nurses of Ontario Standards of Practice.

· Demonstrated ability to analyze clinical data and use quality indicators to support continuous quality improvement initiatives.

· Strong clinical assessment, documentation, analytical, organizational, and problem-solving skills.

· Excellent communication, leadership, coaching, and interpersonal skills.

· Proficient in Microsoft Office applications and PointClickCare (PCC).

· Ability to work collaboratively within an interdisciplinary team while maintaining confidentiality and professionalism.

· Demonstrated commitment to person-centred care, resident safety, continuous quality improvement, and organizational values.

QUALIFICATIONS:

Formal:

· Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).

· Successful completion of an interRAI/RAI-MDS Coordinator certification program (or willingness to complete upon hire with an 18-month commitment).

· Successful completion of required interRAI proficiency testing, or willingness to complete and maintain competency in accordance with organizational requirements.

· Minimum two (2) years' experience in Long-Term Care preferred.

· Experience in resident assessment, care planning, clinical documentation, and interRAI/RAI processes. Previous experience as a RAI Coordinator in Long-Term Care is preferred.

· Working knowledge of the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, InterRAI standards, and College of Nurses of Ontario Standards of Practice.

· Experience with PointClickCare (PCC) or other electronic health record systems is considered an asset.

· Ability to work collaboratively within an interdisciplinary team.

· Must meet the Home's immunization and communicable disease surveillance requirements in accordance with organizational policy and applicable legislation.
