Full job description

Kemptville District Hospital is seeking a Social Worker/Discharge Navigator to join our growing team, on a Permanent Part-Time basis.

As a member of the multi-disciplinary team, the Social Worker/Discharge Navigator supports individualized, safe, and timely transitions for clients from hospital to home or the next level of care. The Social Worker will provide Social Work consultation, assessments and interventions to acute care clients and families to facilitate individualized, safe, and timely transitions home or to the next level of care, ensuring quality and continuity of patient care.

Qualifications

Bachelor’s degree in social work (BSW) or equivalent required.
Current certificate of registration in good standing with the Ontario College of Social Workers and Social Service Workers.
Relevant clinical or discharge planning experience in a healthcare setting.
Certification in Cognitive Behavioral Therapy, Motivational Interviewing, and Mindfulness-based stress reduction, preferred
Ability to work effectively in a multi-disciplinary team environment.
Flexibility to adapt to organizational priorities and client needs.
Demonstrate knowledge and understanding of legislation and acts applicable to patient discharge and transition (e.g. Health Care Consent Act, Mental Health Act, Public Hospital Act, Substitute Decisions Act)
Demonstrated knowledge of financial aid available from federal, provincial, and municipal programs.
Strong communication, organizational, and problem-solving skills.
Knowledge of computer business applications including electronic charting (EPIC).
Respect for individuals of diverse cultures and demonstration of culturally sensitive practice.
