Position: Case Manager, Family Resource Centre
Competition Number: 2026-055

Hours of Work: 37.50 hours per week. Days, Evenings, Weekends.

Reports to: Program Manager, Family Resource Centre

Vacancies: One (1)
Hourly Rate: $ 30.23 – $ 35.36

Job Status: Temporary Full-time (up to Jan 31, 2027)

Union/Non-Union: OPSEU/SEFPO Local 489

Location: Kingston, ON

Job Summary:

The Case Manager, FRC will provide counselling and resources for family members to ensure they can stay well and maintain their own mental health while supporting a loved one on their recovery journey. The incumbent is responsible for providing dedicated support to families of individuals with mental health and/or addiction concerns through supportive counselling, group facilitation, system navigation, and advocacy. As the allied health professional component of the Family Resource Centre, the incumbent will assess the needs of family members and triage individuals appropriately, including determining when FRC volunteers are appropriate to provide support. The incumbent will also provide families with referrals to community resources for individuals who are not connected to AMHS-KFLA services, or those who would be better suited for other supports offered in the community. The FSW coordinates with clinical teams when appropriate to facilitate stronger relationships between clients, families and caregivers.

Minimum Qualifications:

University Degree in a health-related discipline and unrestricted registration with a regulatory body, as applicable.
Three years relevant experience providing services to family members who’s loved one’s live with a serious mental health issues/illness and/or addictions issues.
3 years group development and facilitation experience an asset.
Certification in First Aid and CPR an asset.
Applied Suicide Intervention Skills Training (ASIST) an asset.
Experience working within a case management model, preferred.

Knowledge, Skills and Abilities:

Knowledge of and ability to apply discipline specific principles and practices to supporting family members within a family and client‐centred approach.
Ability to facilitate or certification in family groups including NAMI, Family WRAP, and SMART Recovery (Family & Friends).
Demonstrated in‐depth knowledge of mental health symptoms, issues, psychotropic medications and associated side effects.
Knowledge of issues related to addictions an asset.
Knowledge of and the ability to incorporate best‐practices in community support for individuals living with a serious mental illness and/or addictions.
Demonstrated assessment and crisis intervention skills with broad knowledge of emergency mental health and crisis stabilization. NCVI training an asset.
Demonstrated advanced clinical reasoning and decision‐making skills.
Ability to effectively utilize community resources and supports to meet the needs of family members.
Ability to work under pressure within a changing environment.
Working knowledge of the Health Care Consent Act, Mental Health Act, Personal Health Information Protection Act, and other relevant legislation.
Excellent communication and interpersonal skills in order to effectively engage community services and individuals, increasing awareness of services. This includes an ability to navigate supporting and advocating for families in liaison with clinical teams who support clients.
Well‐developed problem‐solving, prioritization and conflict resolution skills.
Ability to work autonomously as well as collaboratively with multiple stakeholders.
Basic proficiency in computer skills; MS Office preferred.
Ability to work flexible hours, including weekends.
Must possess a valid Ontario Driver’s License and have regular access to a reliable vehicle and provide proof of $2,000,000 vehicle insurance. Incumbent may be required to transport clients.
Required to provide a satisfactory criminal reference check (CRC) with Vulnerable Sector Screening prior to hire.
French Language proficiency considered an asset.
