ABOUT THE HOSPICE OF WINDSOR-ESSEX COUNTY
Founded to provide compassionate, clinical, end-of-life care, The Hospice of Windsor and Essex County has been caring for our community since 1979. Our Hospice believes in equitable access to care for all. We provide specialized wrap-around services to adults, children, and their caregivers, in the setting of their choice – whether it be in our Residences or in their home.
With only partial government funding, it has been through the generous support of our volunteers and our community that we have continued to provide memorable care, meeting the physical, emotional, and spiritual needs of patients and their loved ones.
ROLE OVERVIEW
The Manager of the Hospice Residence is dedicated to Hospice’s vision, mission, and values, and will support and work towards achieving the strategic directions set out by the Board of Directors. They will put their best efforts forward to promote the interests of our patients as well as contribute to a positive work culture.
The Hospice Residence Manager is a strong leader with clinical expertise who will ensure the delivery of exceptional high quality, patient-centered care. Supporting and mentoring nurses, Personal Support Workers (PSW), student learners, and volunteers at the Hospice Residence.
The Hospice Residence Manager is responsible for overseeing clinical practice, quality improvement projects, training, education, mentorship, policy and procedure development, and the completion of required health and safety drills and exercises, or other accreditation, regulatory or legislative requirements. The manager is responsible for supporting staff to meet the unique needs of patients, families and caregivers in managing difficult symptoms, complex ethical decisions, and navigating challenging situations. They facilitate admissions, manage the census and appropriate staffing resources, and facilitate discharges as required. The Hospice residence manager is primarily based in Erie Shores with a counterpart at the Windsor Residence. Coverage will be provided to support the needs of either campus as needed.
PRIMARY DUTIES AND RESPONSIBILITIES
Clinical Program Management

Provide direct leadership and oversight to the interdisciplinary team (Registered Nurse, Registered Practical Nurse, Personal Support Workers, students etc.) to ensure the provision of evidence-based, quality patient care.

Manage the daily operations of the Hospice Residence, ensuring a safe and functioning workplace is maintained. Escalate issues to Facilities/Maintenance and Director as required.

Apply knowledge in palliative clinical practice in the mentorship of all clinical staff.

Collaborate with the Hospice Palliative Physician team and foster an interdisciplinary team environment.

Respond to and manage concerns, complaints, and feedback from patients, families and staff in a timely and professional manner.

Lead chart audits, skills tests or other assessments to ensure staff are compliant with best practices and maintaining competencies.

Ensure seamless communication to staff regarding any updates in the Residences; changes in policies, procedures or standardized practices. Lead Team Meetings or related training as required.

Collaborate with the Coordinator of Volunteers, Social Work, Spiritual Care or other support services to ensure patients and families are fully supported while at the Residence.

Communicate, implement and ensure that Hospice’s responsive and holistic culture is maintained and evident within the Residences.

Participate in regular interdisciplinary and palliative care team meetings and huddles, ensuring appropriate management, referral and response to inquiries.

Participate in internal Hospice committees and/or working groups and attend meetings as required.

Support Medical Residents, Fellows, Nursing Students, and Interns, in the knowledge development and promotion of palliative care practice.

Provide cross coverage and rotate between the two Residences as needed.

Represent Hospice at assigned or requested meetings with community service providers and other stakeholders.
Program Development
-
Promote the sustainable growth and development of the Hospice Residences.
-
Participate as a member of the management team in the development and evaluation of organizational policies, protocols, and procedures to improve patient care and promote cooperative and efficient staff functioning.

Lead process improvements based on evaluations and feedback of departmental programs.
-
Develop program goals and workplan in collaboration with Director; monitor and evaluate completion and performance.

Annually, review and update program dashboard indicators; regularly review results to promote awareness, measure performance and implement process improvements where required, with outcome measures.

Provide oversight for data collection as required for ministry statistics, internal reporting and quality improvement, and provide supporting data and analysis as required. Maintain a thorough understanding of data requirements for funding, internal performance indicators and special projects.
Team Management

Act as liaison between team members and Directors/Leadership Team; communicate and lead change management and escalate concerns when appropriate.

Oversee and ensure the completion of the staffing schedule by the Hospice Residence Administrative Assistant; manage and approve time off requests.

Ensure the staffing schedule is posted for staff accessibility on a consistent basis.

On rotating basis, provide on-call support for the Hospice Residences to address urgent concerns from staff, patients or families. In emergent situations, the manager may be required to cover unfilled shifts to maintain staffing requirements.

Communicate needs for medical supply ordering to Administrative Assistants.

Provide leadership and mentoring for team members; support goal setting and team/role development.

Manage performance for all team members and initiate progressive discipline as required.

Complete annual performance evaluations for all team members, and probationary period evaluations for new hires.

Encourage staff to participate in employee engagement initiatives and provide feedback to contribute to program development and quality improvement projects.

Identify education needs of Hospice Residence staff and collaborate with administrative team to implement.
-
Participate in the recruitment and selection process for roles within the team and other related roles within the organization as required.
ACCOUNTABILITIES

Accountable to the Director of Patient and Family Clinical Services.

Accountable for performing in adherence to the Hospice Mission, Vision and Values.

Accountable for adhering to Hospice Policies, professional behaviour and for ensuring review of such as required.

Accountable to the College of Nurses of Ontario standards of practice, and to maintain clinical competence through continuing education and required reflective practice protocols.

Accountable to submit annually to Human Resources proof of current certificate of competence registration from the College of Nurses of Ontario.

Accountable for ensuring adherence to HPCO and CARF accreditation standards.
REQUIRED QUALIFICATIONS AND EXPERIENCE

GENERAL REQUIREMENTS
-
Legally entitled to work in Canada.
-
Valid Police Record Check including vulnerable sector check (within six months of hire, and declaration of offences as required).

Valid drivers’ license and reliable means of transportation.
-
French language skills are an asset.
-
This position will require flexing hours to address staffing needs, attend meetings, conferences, professional development opportunities, education and training, and coordinating services including possible weekends and holidays.

TECHNICAL REQUIREMENTS
-
Proficient in computer use including Microsoft Office (Word, Excel, Outlook, Teams), electronic health records. Familiarity with Cerner/Oracle EMR is an asset.

Expert knowledge of interdisciplinary patient care, infection control, pressure injury prevention, fall risk prevention, medication management and proper body mechanics in the provision of care.

Expert proficiency in pain and symptom management.
-
Strong ethical and compassionate approach; ability to adapt to changing situations.
-
Demonstrated teaching and training skills in both one-on-one and group settings.
-
Physical ability and stamina to provide clinical care to residents throughout a 12-hour shift.

EXPERIENCE
-
Management or Leadership training and/or direct experience managing clinical personnel preferred.
-
At least five (5) years’ experience in oncology and/or community palliative care preferred.
-
Experience with quality improvement, accreditation and regulatory compliance is an asset.

EDUCATION AND CERTIFICATIONS
-
Registration in good standing with the College of Nurses of Ontario as a Registered Nurse is required.
-
Masters Degree in Nursing preferred.
-
Certificate in the “Fundamentals of Palliative Care” and “Comprehensive Advanced Palliative Care Education” (CAPCE) courses is required.
-
Current Canadian Nurses Association (CNA) certification in Hospice Palliative Care is an asset.
-
CPR Certification and annual maintenance is required.
