Job Overview

About Us

The Sarah Tapley Foundation supports families navigating critical childhood illness. Carrying on Sarah's legacy, we provide transformational, high-impact support to families facing their most challenging moments. Working closely with social workers at leading hospitals including SickKids and McMaster Children's Hospital, we come alongside families with both practical resources and genuine care during times of crisis.

At the heart of the Foundation is relationship. The trust we've built with hospital partners flows directly from the compassionate, personal way we walk with families. This role exists to protect and carry that heart.

Position Summary

We are seeking a warm, emotionally intelligent, and deeply compassionate Family Care Liaison to serve as the Foundation's primary point of personal connection with the families we support.

This is the relational core of our work: listening to families, understanding their needs, coordinating their care, and ensuring they feel seen and supported through the hardest season of their lives. The successful candidate will bring genuine pastoral sensitivity, discretion, and the emotional capacity to hold difficult stories with grace.

Key Responsibilities

Family Care & Relationship (60–70% of role)

Serve as the primary personal contact for supported ("hero") families
Conduct intake calls and assess family needs with sensitivity and care
Provide ongoing emotional support and regular check-ins throughout the support period
Listen well, offering families a safe space to process their experiences
Walk with families through crisis, loss, and uncertainty with compassion and discretion

Care Coordination (20–30% of role)

Coordinate practical support and care packages in partnership with community resources

(e.g., CityServe)

Liaise with hospital social workers to receive referrals and confirm family needs
Implement board-approved support plans, communicating clearly with families about what the Foundation is providing
Where appropriate, gently invite families to share their stories, always honouring their comfort and privacy
Report family updates and outcomes back to the Senior Development Officer and Board

Administrative Support (5–10% of role)

Maintain accurate, confidential records of family interactions and support provided
Coordinate with the Finance Clerk and Senior Development Officer on disbursement details
Contribute family insight to communications and storytelling efforts, within consent boundaries

Required Qualifications

Demonstrated experience in a caregiving, pastoral, counseling, social work, or family-support capacity
Exceptional interpersonal and active-listening skills
Genuine empathy and emotional intelligence, with the ability to build trust quickly
Strong verbal and written communication skills
Discretion and a firm commitment to confidentiality
Ability to work independently and manage sensitive situations with maturity

Preferred Qualifications

Background in pastoral care, counseling, chaplaincy, social work, or a related field
Personal or professional familiarity with the experience of childhood illness or family crisis
Experience working with hospital social workers or healthcare partners
Familiarity with community support resources in the Niagara / GTA region

Working Conditions

Part-time, 5–10 hours per week, flexible to family caseload

Primarily remote, with phone and virtual contact as the main mode of connection
Occasional in-person family visits, deliveries, or events in the Niagara / GTA area
Emotionally demanding work involving families in critical and sometimes end-of-life circumstances

Wellness & Support

The Foundation recognizes the emotional weight this role carries. To support the wellbeing of the person in this position, a monthly counselling/wellness benefit ($400/month) is provided, along with intentional board-level care, boundaries around caseload, and a supportive team environment.
