Full job description

Join a collaborative, team‑based care environment where your role truly matters.
As a permanent part-time Nurse Practitioner in our Community Health Centre, you’ll work as part of an interprofessional team that values shared decision‑making, mutual respect, and wraparound care. You’ll collaborate closely with physicians, other nurse practitioners, nurses, social workers, dietitian, and other allied health professionals to support patients holistically—addressing physical, mental, and social determinants of health. This is an opportunity to practice to your full scope in a supportive, mission‑driven setting where teamwork is at the heart of everything we do.

Reporting to the Operations Manager, and under the guidance of the Physician(s), the Nurse Practitioner will provide primary care services to individuals and families, emphasizing a holistic approach to chronic disease management, health promotion, disease prevention, support and treatment.

Duties and Responsibilities

Provide primary care to individuals, and their families by taking into account risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client and, when applicable, with their families.
Perform clinical duties, assessing clients for routine health and illness care.
Develop/initiate health teachings and screening programs.
Conduct initial screening assessment, provide appropriate care and/or make appropriate referral within the scope of practice of a Nurse Practitioner.
Ensure the patient receives the right care from the right professional at the right time.
Identify health needs of the clients and various other community groups.
Work with SGBCHC staff to plan, implement and evaluate programs to best suit the needs of the clients within the community.
Support the administration and operations of the health center by identifying areas where the development of protocols and procedures is needed to improve client services.
Promote effective staff functioning by participating in committee work and staff meetings.
Develop and implement charting tools, keeping accurate medical records and statistics.
Participate in chart reviews and discussion of problem cases.
Mentoring and training medical and nursing students as needed.
Advocates for clients as warranted and facilitates connections between clients and community supports in order to assist clients in accessing required resources and in following up with any funding issues.
Works as a member of an inter-professional/multi-disciplinary team and in collaboration with agencies/services in the community, and family members, wherever appropriate, for the purpose of consolidating and coordinating client treatment.
Establishes therapeutic relationships with clients, their family members/caregivers/SDMs in order to build trust and effectively deliver services.
Maintain a level of care that meets holistic and ethical standards based on the College of Nurses requirements.
Protects client privacy and confidentiality in accordance with legislation, CHC policies and standards established by CNO.
Maintains professional competence and knowledge of current practices by participating in professional development and on-going research.
Uses sound professional judgment and acts in accordance with all CHC policies and procedures and standards of practice set forth by CNO.
Other duties as assigned

JOB REQUIREMENTS

Education:

Registration in good standing with the College of Nurses of Ontario as a Registered Nurse Practitioner
Master's level degree in nursing

Knowledge/Experience:

Experience working in a community health care setting an asset.
Knowledge of funding agencies available to assist clients in accessing needed support services and community resources.
Practical knowledge and understanding of pertinent legislation.
Knowledge of and experience working with diverse and marginalized populations and those with mental illness is an asset.

Skills:

Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability, demonstrating empathy and compassion
Effective listening, observation and facilitation skills to assess the client and provide appropriate treatment and consultation in an inclusive manner.
Ability to deal with demanding interpersonal situations and respond with good judgment, compassion and understanding.
Ability to analyze and independently problem solve using a health equity lens.
Ability to engage with clients, staff and community partners in a virtual environment using appropriate technology supported by SGBCHC.
Strong collaboration skills to manage service delivery in conjunction with case managers, clients, their family members/caregivers/SDMs and other health care professionals.
Strong mediation, negotiation and conflict resolution skills to respond or assist in emergency situations.
Commitment to practicing infection and prevention control policies, adhering to pandemic precautions in the workplace.
Valid Ontario driver’s license and use of own vehicle for CHC business

Job Types: Part-time, Permanent

Pay: $56.01-$64.35 per hour

Expected hours: 18.75 per week
