The Lead, Clinical Consultant Pharmacists (CCP) provides day to day clinical, and operational, support to a team of CCPs supporting long-term care (LTC), retirement homes and other senior care settings across Western Ontario. This role ensures high-quality, consistent, and compliant clinical and consultant services aligned with Ontario regulations, organizational standards, and best practices in geriatric pharmacotherapy and medication management safety.

SPECIFIC RESPONSIBILITIES
- Clinical Leadership & Quality Oversight
- Provide clinical guidance to CCPs in their consultant core tasks, resident core tasks, and quality/program initiatives with always a resident-centred care perspective.
- Ensure CCP practice aligns with Ontario College of Pharmacists (OCP) standards, Ministry of Long-Term Care (MLTC) regulations, and organizational policies.
- Support complex clinical decision-making, including high risk medication regimens as well as high risk processes.
- Review clinical documentation of tasks, audits and interventions for quality, consistency, and appropriateness.
- Lead and support completion rates of core tasks, quality improvement initiatives, and corrective action plans (when applicable).
- Promote evidence-based practice and continuous clinical learning.
- Operational & Program Management
- Supports the day-to-day CCP operations, to support consistent service delivery across multiple sites.
- Provide input to support scheduling, portfolio balancing, and prioritization of CCP activities.
- Support timely completion of required tasks, reports, documentation, and regulatory deliverables.
- Collaborate with internal teams to align clinical services with organizational goals.
- Identify inefficiencies and provide process improvement inputs to enhance workflow and productivity.
- Support implementation and optimization of clinical tools, technology, and documentation systems.
- Stakeholder & Relationship Management
- Serve as a point of escalation for clinical or service-related issues involving CCPs.
- Build and maintain strong relationships with regional corporate clients, LTC homes, Directors of Care, medical directors, nurses, and interdisciplinary teams.
- Support consistent messaging and expectations with external partners and clients.
- Change Management & Strategic Support
- Support change initiatives such as new clinical programs, regulatory and policy updates, or workflow standardization.
- Communicate changes clearly and effectively to CCPs, ensuring understanding and adoption.
- Provide frontline feedback to VPCO/senior leadership to inform strategic planning and program evolution.

TECHNICAL COMPETENCIES
- Registration as a Pharmacist in good standing with a provincial licensing College
- Minimum 5–7 years of experience in long-term care as a Clinical Consultant Pharmacist or geriatric pharmacy practice.
- Prior leadership, team lead, or senior clinical role strongly preferred.
- Experience supporting large clinical teams considered an asset.
- Comfort working with Excel and PowerBI dashboards
