Title: Rooming House Case Manager

Location(s): 401 Bell St. South Ottawa, ON

Salary $53,633.18- $60,705.12 per annum **based on work experience

Hours: 40-Hour Work Week (35 hours plus 5 hours allocated to daily half hour paid lunches and unpaid breaks)

About the role:

Working under the direction of the Housing and Reintegration Coordinator, you will support individuals living in rooming houses who are experiencing housing instability or are at risk of homelessness. Using a coordinated case management process and a Housing First approach, you will help clients access services, address barriers and maintain stable housing.

You will join an established Housing and Reintegration team that values collaboration, shared problem-solving and mutual support. While another Rooming House Case Manager provides specialized support to seniors, this position will work with rooming house residents across a broad range of ages and needs. Referrals may come from community partners, shelters, hospitals and other service providers throughout Ottawa.

As the successful candidate, you have:

Postsecondary education in social work, social services or a related field, or an equivalent combination of education and relevant experience
A minimum of two years of experience supporting individuals with complex needs, including people experiencing homelessness, mental health challenges and/or substance use concerns
Knowledge of Housing First, harm reduction, trauma-responsive and client-centred approaches
Knowledge of the Residential Tenancies Act, 2006 and the rights and responsibilities of tenants and landlords
Familiarity with HIFIS, SPDAT or other housing assessment and case management tools
An understanding of Ottawa’s housing and homelessness service system, including housing placement resources and processes for individuals who face significant barriers to obtaining and maintaining housing
Demonstrated ability to build respectful, professional relationships with clients, landlords, community agencies and other service providers
Strong communication, negotiation and advocacy skills
Excellent interpersonal skills and the ability to work both independently and collaboratively
Sound judgment and the ability to respond calmly and effectively to challenging or urgent situations
Strong organizational, documentation and time-management skills
The ability to prepare clear case notes, reports and professional correspondence
Proficiency with Microsoft Office, including Word, Outlook and Excel, as well as experience working with electronic databases
A valid Ontario Class G driver’s licence and access to an insured personal vehicle for regular travel throughout the City of Ottawa
The ability to work flexible hours when program or client needs require it
The ability to provide a satisfactory Police Record Check for the Vulnerable Sector
Bilingualism in English and French is considered an asset

You Will Be Prepared To:

Meet clients where they are, including in their homes and at community locations that are accessible and comfortable for them
Complete intake, triage and assessments to identify housing history, barriers to housing stability, level of need and immediate priorities
Work in a variety of community and residential environments where pests, including bed bugs, may be present, while following all organizational health and safety procedures and using the appropriate personal protective equipment provided
Work collaboratively with clients to develop individualized, client-led plans that reflect their housing and personal goals
Support clients with immediate and ongoing needs related to identification, income, food, clothing, shelter, health care and other essential services
Assist clients in locating and securing appropriate housing through active housing searches, applications and property viewings
Build and maintain positive working relationships with landlords and promote the benefits of participating in a Housing First program
Help clients understand tenancy expectations and develop the skills and supports needed to maintain their housing
Provide ongoing case management, practical assistance, coaching and follow-up based on each client’s identified needs
Provide crisis intervention, safety planning and service coordination when required
Connect clients with JHS Ottawa programs, supportive housing options and other appropriate community services
Advocate with housing providers, income supports, health services and other systems when needed
Monitor client progress and adjust service plans collaboratively as needs and circumstances change
Coordinate or participate in case conferences with clients, colleagues and community partners
Maintain regular contact with clients while respecting client choice, privacy and appropriate professional boundaries
Complete timely and accurate case notes, incident reports, statistical records and other required documentation
Contribute meaningfully to staff meetings, case consultations and program planning
Follow all organizational policies and procedures related to client and staff safety, confidentiality and professional conduct
Deliver services through a gender responsive, trauma responsive, harm reduction and anti-oppressive framework

Work collaboratively with a broad range of community organizations, rooming house services, housing providers and emergency services
Travel regularly between client homes, community locations and JHS Ottawa sites
Adapt effectively to changing priorities and the varied needs of clients and the program
