Job Summary
The West End Integrated Falls Prevention Program aims to address falls prevention strategies and treatment, improve mobility, and enhance overall quality of life for seniors living in the community and their caregivers. Under the direct supervision of the Physiotherapist and Occupational therapist, the Rehabilitation Assistant will assist in delivering evidence pasted interventions aimed at improving strength, balance, mobility, functional independence, and home safety for older adults.
Job Specific Responsibilities:
Clinical Care - Under the direction of Physiotherapists and Occupational Therapists, the Rehabilitation Assistant will:
- Support clients in improving mobility, balance, strength, and endurance through structured exercise programs
- Assist clients with gait training, transfers, and safe ambulation techniques
- Support occupational therapy interventions focused on daily living skills
- Reinforce home safety strategies and fall prevention education
- Assist with training and education on the safe use of mobility aids and adaptive equipment
- Observe, monitor, and document client responses to interventions
- Complete accurate, timely, and professional documentation of delegated interventions
- Communicate changes in client status, safety concerns, and progress to clinical staff
- Participate in program planning
- Assist in conducting group exercise and educational sessions
- Support delivery of falls prevention workshops, clinics and community outreach programming
- Demonstrates appropriate and effective written, oral and non-verbal communication with clients, caregivers, colleagues and the general public
- Participate in interdisciplinary team meetings and case discussions
Support
- Maintain cleanliness, safety, and organization of rehabilitation and exercise equipment
- Identify and report faulty, unsafe, or missing equipment
- Support setup of treatment spaces and community program locations
- Ensure safe physical environments for client participation

- Contributing to the Centre's work by participating in meetings and committees.
- Working during both regular and extended hours of operation in locations identified by the Centre.
- Contributing to the Centre's practices of hiring, orienting and training of staff.
- Contributing to the Centre's efforts to secure and maximize resources for current and new programs, services and activities.
- Participating in the Centre's efforts to enhance its capacity through staff development.
- Working in a manner that supports the Centre's Occupational Health and Safety policies and procedures.
Qualifications:
- Diploma in Physiotherapy Assistant (PTA) and/or Occupational Therapy Assistant (OTA) from a recognized institution.
- Current CPR and First Aid certification required
- Experience working with older adults and/or rehabilitation clients
- Experience in community-based health care or outpatient rehabilitation strongly preferred
- Demonstrated ability to work under the direction of a regulated health professional within a supervised clinical framework
- Experience supporting individuals at risk of falls considered an asset
- Strong interpersonal, communication, and teamwork skills
- Ability to work independently in community settings with minimal supervision
- Strong organizational and time management skills
- Competency in basic computer systems and electronic documentation
- Commitment to client-centred, strengths-based care
- Ability to adapt to changing priorities in a dynamic environment
- Valid driver’s licence, access to a reliable vehicle, and appropriate insurance required
- Physical ability to support mobility, transfers, and active client participation
- English required; bilingualism (French and/or other languages) is considered an asset
