Key Responsibilities
- Conduct one-on-one wellness and social visits with seniors living independently in the community
- Build positive, supportive relationships with clients and promote social engagement and wellbeing
- Plan, coordinate, and facilitate weekly group events, activities, and wellness programs for clients
- Collaborate with recreation coordinators and community partners to develop meaningful programming opportunities
- Support clients in accessing community resources and participating in social and recreational activities
- Monitor client engagement and wellbeing, and communicate relevant updates to the Seniors Navigator and team
- Assist with program setup, activity preparation, transportation coordination, and event logistics
- Maintain accurate documentation and uphold client confidentiality and professional standards
- Complete accurate documentation, evaluations, and participation records, and use data to support program development and continuous improvement.
- Support a strong culture of safety by following all standard operating procedures, safe work practices, and contributing to a safe environment for clients, staff, volunteers, and students.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- Availability to work Monday – Friday
- Experience working with older adults in community, healthcare, recreation, or wellness settings
- Possession of a valid driver’s license and access to a reliable vehicle for travel to client’s homes
- Ability to provide a clear criminal record and vulnerable sector check required upon hire;
- Current First Aid & CPR is required;
- Demonstrated passion for supporting the social well-being and quality of life of older adults
- Excellent interpersonal, communication, and relationship-building skills.
- Strong organizational skills and the ability to work independently and collaboratively within a team environment
- Commitment to representing the Nursing Home Without Walls program in a professional and compassionate manner
- Commitment to client-centered, respectful, and culturally sensitive care
- Demonstrated ability to adapt programs to changing client needs and behaviours
- Compassion, patience, and an empathetic approach to client care and support
- Creativity and enthusiasm in planning engaging activities and events
- Experience working in both one-on-one and group settings
- Knowledge of senior wellness, healthy aging, and community-based supports
