We are currently recruiting for Social Support Coordinators - two (2) Temporary Full-Time positions:

Position 1: Temporary Full-Time until December 31, 2027, with the possibility of extension.
Position 2: Temporary Full-Time with the end date to be determined.

We are seeking an enthusiastic, innovative leader to join our Social Support (S.S.) Coordinator team. Reporting directly to the Manager, Good Life Professional Hub services team. The S.S. Coordinator is responsible for supporting the participants to overcome barriers and realize their goals and dreams through a person-directed planning framework, ensuring the programs chosen by participants are implemented as designed.

The Social Support Coordinator will collaborate with the BAC Managers in the oversight of the day-to-day delivery of programming at BAC and provide one-to-one social support to our participants. They will be responsible for reporting key metrics on monthly programming outcomes to both the Director of Good Life and the Director of Community Living.

Other duties include, but are not limited to, supporting outreach assessment planning, conducting participant evaluations on programming, and communicating participant feedback; coordinating tasks with staff on program delivery; educating staff on supported decision-making delivery and social support model; and fostering a healthy, safe, and respectful person-centered environment. This position may include evening and weekend hours.

The Ideal Candidate

Bachelor’s degree in social sciences and/or health-related field (i.e., Degree in Psychology, B.Ed., Social Worker, Registered Nurse, or Licensed Practical Nurse (diploma), or diploma related to Disability Support Services along with a minimum of three to five years’ experience working and supporting individuals. An equivalent combination of education and experience may be considered. Supervisory experience would be considered an asset.
Strong understanding and value for advocacy for a human rights approach to support and commitment to supporting individuals with disabilities within a Social Model based on the UN Convention on the Rights of Persons with Disabilities.
Facilitate discussions with participants and families to create a vision of a good life: (i.e., What would a good life mean to you? What is important to you? Imagine if? What worries you?)
Community navigation skills and relationships with other organizations and stakeholders foster opportunities for inclusion, develop natural networks of support, and enable participants to experience flexible mainstream services such as education, health, leisure, transportation, etc.
The ability to think creatively, and outside the box to generate solutions to barriers to inclusion and a good life.
