Full job description

Position Summary

This position is a front-line member of the Yellowknife Women's Society Housing First team. Reporting to the Housing First Manager, you will provides client-centred case management, advocacy, and coordinated supports to individuals experiencing or at risk of homelessness.

The Service Coordinator collaborates with internal programs, community agencies, and healthcare partners through a coordinated access, Circle of Care, and case conferencing model to promote housing stability, wellness, independence, and successful community integration. Services are delivered using trauma-informed, harm reduction, person-centred, and culturally safe practices.

Key Responsibilities

Client Support & Case Management

Complete client intake, including PATH assessments and other program screening tools.
Collaborate with clients to develop, implement, monitor, and regularly review individualized support plans based on client goals and housing stability.
Provide ongoing case management, advocacy, crisis intervention, and practical support using trauma-informed and harm reduction principles.
Coordinate referrals and facilitate access to healthcare, addictions treatment, mental health services, income supports, employment, education, legal services, and other community resources.
Participate in Coordinated Access meetings, Circle of Care planning, and multidisciplinary case conferences to ensure coordinated service delivery.
Advocate on behalf of clients while encouraging self-determination and independence.
Support clients in budgeting, financial planning, Income Assistance applications, taxes, and other financial documentation.
Assist clients in resolving barriers with landlords, government agencies, and community service providers.
Organize and facilitate life skills, educational, recreational, cultural, and wellness activities.
Support clients in preparing for employment, volunteer opportunities, or education when appropriate.

Documentation & Administration

Maintain accurate, timely, and confidential client documentation using HIFIS, Apricot, and other approved documentation systems.
Complete case notes, assessments, reports, statistics, referrals, and other required program documentation.
Ensure documentation meets organizational, territorial, and funding requirements.

Team Collaboration

Work collaboratively with Housing First staff, YKWS programs, healthcare providers, Indigenous organizations, and other community partners.
Participate in regular team meetings, case conferences, supervision, and ongoing professional development.
Participate in an on-call rotation in accordance with departmental schedules. Staff are expected to respond to urgent client situations during assigned on-call periods. Compensation for on-call responsibilities will be provided in accordance with organizational policy.

Safety Responsibilities

Follow all Yellowknife Women's Society policies, procedures, and health and safety protocols.
Maintain professional boundaries while fostering respectful and supportive relationships with clients.
Conduct community visits and home visits in accordance with organizational safety procedures, including not entering client residences alone unless approved or risk assessed.
Immediately report incidents, safety concerns, hazards, and critical occurrences according to organizational procedures.

Qualifications

Diploma or degree in Social Work, Human Services, Psychology, Indigenous Studies, Community Rehabilitation, or a related field is preferred. An equivalent combination of education and relevant experience may be considered.
Experience providing case management, outreach, or support services to individuals experiencing homelessness, mental health challenges, substance use, trauma, or other complex barriers.
Demonstrated understanding of Housing First principles and client-centred service delivery.
Knowledge of trauma-informed, person-centred, harm reduction, and culturally safe practices.
Understanding of the social determinants of health, homelessness, housing stability, and social justice.
Knowledge of the impacts of colonialism, residential schools, and intergenerational trauma, with the ability to work respectfully alongside Dene, Inuit, Métis, and other Indigenous peoples, incorporating Elders, cultural knowledge, and traditional supports when appropriate.
Experience completing assessments, developing individualized support plans, and maintaining accurate client documentation using electronic case management systems such as HIFIS, Apricot, or similar databases.
Strong crisis intervention, conflict resolution, advocacy, and de-escalation skills.
Excellent interpersonal, written, and verbal communication skills.
Strong organizational, time management, and documentation skills with the ability to manage competing priorities.
Proficiency with Microsoft Office and electronic record management systems.
Valid Class 5 Driver's Licence is required.
Recent Vulnerable Sector Check are required.

Working Conditions

Flexible schedule, including evenings, weekends, and participation in an on-call rotation as required.
Frequent community outreach, home visits, and transportation of clients.
Exposure to unpredictable and high-risk situations requiring sound judgment and de-escalation skills.
