Job Description

JOB SUMMARY:

Case Aides work closely with the Sagamok Anishnawbek team of Band Representatives and frontline workers, as well as with the Sagamok Anishnawbek families. Case Aides are responsible for supporting front line workers with a range of responsibilities during all stages of a protection case (investigation and assessment to ongoing case management). They can also provide direct support to families without Band Representatives present, including activities that help carry out the wellness plan for the child and family.

The Case Aide works out of the Child and Family Advocacy Unit and reports directly to the Child and Family Advocacy Unit Manager for their daily job duties. The Family Support Worker is expected to conduct their responsibilities in a way that promotes the culture and aspirations of Sagamok Anishnawbek.

DUTIES & RESPONSIBILITIES:

1. Support to Sagamok Anishnawbek Frontline Workers – provide assistance, and support to Sagamok Anishnawbek frontline staff as they work with families who are in contact with the protection services, or at risk of becoming involved with protection services. This includes:

Accompany frontline workers to home visits
Attend case conferences and other meetings
Attend and support family circles/conferences
Assist the frontline workers with compiling and uploading information for various recordings and documentation into the appropriate system, including assessments, reports, etc.
Maintain an up-to-date case log and record accurate minutes/records of meetings, visits, case conferences, etc.
Keep Child and Family Advocates apprised of all visits on a regular basis
Prepare reports, statistics, briefing notes and correspondence

2. Support to Sagamok Anishnawbek Families – provide direct supports to families in support of their wellness plan, working with them to understand their challenges and build on their strengths. This includes:

Make referrals to community based and culturally appropriate prevention resources and services, as well as to diagnostic services as needed
Provide more intensive family support services as needed to promote physical, mental, emotional and spiritual wellbeing (e.g., deliver life skills training; provide counselling supports; connect families to financial resources for home repairs, education costs, rental fees, etc.; schedule appointments; transport families to appointments/visits; deliver after hour/on-call services; etc.)
Provide transportation services to children, youth and parents. This includes transportation to scheduled events, appointments and/or activities, as well as to access visits, and emergency transportation.

3. Liaison and Coordination

Provide referrals and letters to agency collateral service providers
Communicate and follow up with service providers to obtain updates
Provide advocacy and liaison services with other service providers and agencies to promote and support client efforts and to ensure service needs of families are addressed

4. Other

Participate in cultural awareness and cultural sensitivity training
Collaborate on the planning and implementation of community events to promote personal, family and community wellness
Deliver additional targeted activities that advance Sagamok’s wellness priorities related to at risk children and families, as assigned by the Child Family Unit Manager (i.e., the worker may be assigned specific portfolios of work such as post-majority care, kinship care, etc.)
Input into Sagamok Anishnawbek’s Koognaasewin efforts to assert jurisdiction for child welfare, including: input into transformation and visioning exercises; recommend promising/best practices to explore; provide feedback on potential structures, processes and intervention responses; develop policies and procedures; support engagement activities related to Koognaasewin; etc.

TERMS AND CONDITIONS OF EMPLOYMENT:

Hold and willing to secure, CPR and First Aid Certificate.
Must have a valid Ontario driver’s license and access to a vehicle with appropriate liability insurance.
Able to travel on short notice.
Ability to work flexible hours on short notice.
Sign and comply with an annual Oath of Confidentiality.
Must provide a clear Criminal Records Check (CPIC) valid within 6 months prior to commencing employment.
Subject to six months’ probation.

QUALIFICATIONS:

Minimum academic requirement is college diploma in Human Services.
Minimum 2 years experience working in a Child Protection environment and working directly with children and families.
Knowledge of child development, individual and family counselling techniques.
Knowledge of the Child, Youth and Family Services Act, and other relevant legislation.
Proficient in Microsoft Word, Excel, PowerPoint, internet and electronic email.
Ability to understand and speak Ojibwe will be considered a definite asset.
Local knowledge and understanding of Sagamok Anishnawbek’s services and will be considered an asset.
