Position Summary
This is an exciting opportunity for an innovative and experienced Registered Nurse (RN) to join the Feeding & Swallowing and Brain Injury Rehab/Concussion teams. The RN in this role will be part of establishing a nursing role in the feeding & swallowing team.
The Ambulatory Care Nurses (ACN) skill set is grounded in rehabilitative principles of assessing, managing and evaluating complex health conditions in collaboration with the interprofessional team and client/family. They partner with the client and family, to establish plans of care to obtain optimal health and wellbeing, utilizing a strength-based approach. The ACN has unique skills required to help clients and families navigate an ever-changing complex health system. They are also required to provide complex procedural care, support clients/families in decision-making, and provide client education and coordination of services.
The RN is registered with College of Nurses of Ontario and works to full scope of practice within the current legislative framework and based on the hospital policies and standards of care.
Key Responsibilities
Clinical Practice
- Uses evidence based practice to plan, implement and evaluate nursing care for clients within the ambulatory brain injury rehab, concussion and feeding & swallowing clinics.
- Provides nursing care utilizing a strengths based approach that promotes and supports the physical, psychosocial, and spiritual well-being of clients and families
- Provide education and support to patients and their families about treatment plans, and lifestyle modifications during and between clinic visits.
- Monitor client’s progress, and provide support to the client and family in implementing recommendations.
- Serve as a point of contact for patients and families, coordinating with other team members and ensuring seamless care delivery.
- Support physicians/Nurse Practitioner in making referrals to other team members as needed.
- Practices culturally competent care
- Deliver community education initiatives, workshops, or support groups related to restrictive eating and neurodevelopmental disorders in collaboration with the team
- Collaborates with clients/families, members of the interprofessional team and community health care providers to ensure an optimum and integrated approach to care
- Bases practice on nursing science and content relevant to nursing from the sciences and humanities and established professional practice standards
- Demonstrates a spirit of inquiry by examining current practice and identifying questions for studies.
- Provides nursing care that promotes and supports the physical, psychosocial, and spiritual well-being of clients and families
- Supports the maintenance of a safe and healthy work environment and advances a culture of client/patient safety through work and daily practices
- Supports and respects the client / family’s rights to make decisions based on their own values, beliefs and experiences
Professional Development/Leadership
- Regular check-ins with Nursing Team to support with ongoing scheduling, onboarding, and workload planning
- Contributes to the development of professional nursing practice through participation and leadership in unit
- Demonstrates a commitment to lifelong learning and continuous development of self by identifying learning needs and establishing annual goals and objectives
- Maintains own knowledge, skills, and abilities through education, and training both in-house and in post-secondary institutions
- Manages change in an inquisitive and client centered manner
- Ensures that nursing practice issues are addressed using appropriate lines of communication within the organization
- In partnership with clients/families and the interdisciplinary team, provides client and family-centred care which is timely, goal directed, evidence based and outcome oriented
- Supports and respects the client/family’s rights to make decisions based on their own values, beliefs and experiences
Research
- Demonstrates and utilizes the teaching / learning process to provide education, both formal and informal to clients / families, colleagues, students and other professionals within a fully affiliated academic healthcare environment
- Shares new knowledge and best practices with colleagues and other professionals within the organization and the community
- Participation in quality initiatives and measurements
Education
- Currently registered as an RN with the College of Nurses of Ontario
- BCLS certification upon hire
- Previous Paediatrics nursing experience, rehabilitation nursing experience preferred
Experience
- Must have a minimum of one year of nursing experience in a pediatric environment.
- Experience, knowledge or interest in an ambulatory care nursing role.
Certification/Licenses
- Current certificate of competence by the College of Nurses of Ontario
- Membership in RNAO preferred
- Current BCLS certificate—Basic Rescuer
- Certification in Pediatric Nursing from Canadian Nurses Association is preferred
