Program Summary:
The
Specialized Assisted Living - Sherbourne & Respite program delivers individualized support services to people through 8 high-support, permanent residential units in addition to 3 units allocated to the Respite Program. These units alleviate pressure on other healthcare services by offering support for people to transition from hospital settings to the community. Clients accessing both programs may have complex mental health and substance use challenges. The Respite Program offers temporary relief for caregivers or individuals who need intensive support. These short-term, goal-oriented stays provide a consistent space for respite clients to focus on their recovery, which can be related to mental health, substance use, physical health or eviction prevention goals.
The program provides client-centered, individualized services using the principles of Psychosocial rehabilitation. The program offers high-support services and will be staffed with a minimum of 2 staff 24/7. Specialized Assisted Living – Sherbourne and Respite Program operates out of a 3-story site, in downtown Toronto. The property provides single rooms, with shared common spaces (kitchen, washrooms and living spaces). The units on the main floor are accessible, with a ramp to enter the building. Clients can access washroom facilities on the second floor using a stairlift. Respite units will be furnished for clients to allow for a smooth transition to short-term, intensive stays.
Job Summary:
Through a team oriented professional approach, the position provides support to the client to increase their independence, dignity and self-respect, in a safe and secure environment by working within a team-based environment, you will support clients in achieving greater independence, dignity, and well-being within a safe and supportive housing setting.
What You Will Do:-
Client Support & Skill Building: Build strong, respectful relationships with clients, providing coaching and hands-on support in daily living activities such as personal care, medication management, meal preparation, budgeting, and maintaining a healthy lifestyle.

Program & Household Operations: Facilitate house meetings and group activities, support client engagement, assist with daily household operations, and ensure a safe, clean, and well-maintained living environment.

Community & Service Coordination: Help clients navigate healthcare and social service systems, maintain community partnerships, and support or accompany clients to appointments as needed, including participation in 24 hour crisis support.

Conflict Resolution & Recovery Support: Promote a psycho-social rehabilitation / recovery-oriented approach, including harm reduction, conflict resolution, and problem-solving to support positive shared living environments.

Administrative & Team Collaboration: Maintain accurate documentation, contribute to program planning and evaluation, support team communication, and assist with scheduling, training, and continuous improvement initiatives.
What You Bring:-
A relevant degree/diploma in human services or a related field.

At least 2+ years of experience working with the vulnerable populations.

Knowledge of the challenges faced by clients including those experiencing mental health issues mental health, addictions, cognitive impairments, HIV/AIDS etc.

Strong understanding of harm reduction and psychosocial rehabilitation principles.

Strong crisis intervention, problem-solving, and conflict resolution abilities.

Ability to build therapeutic relationships and collaborate with clients on goals and care plans.

Experience working within multidisciplinary teams and facilitating group activities or meetings.

Excellent interpersonal communication, and counselling skills to support clients in sensitive and complex situations.

Effective organizational, time management, and analytical skills.

Proficiency in computer systems, documentation, and record-keeping.

Will be required to work in resident rooms and may be exposed to second-hand smoke.
- Required to be on after-hours on call, on a rotating schedule.
- Walk up and down stairs, physically able to assist client with daily living activities

10%-20% travel to different locations with transit and/or own vehicle, outdoor travel,
- Lifting objects up to 50 lbs.

This position requires working onsite five days per week, Tuesday through Saturday, from 9:00 a.m. to 5:00 p.m., subject to operational requirements need.
