Job Summary

Sagamok Anishnawbek is seeking a compassionate and organized Intake Case Management Worker to join the Mental Health and Addictions team within the Community Wellness Department.

This role serves as the first point of contact for individuals and families seeking services and supports. Through a comprehensive intake and case management process, the Intake Case Management Worker provides client-centred, strengths-based support that promotes recovery, stabilization, and independent living. Working as part of a multidisciplinary team, this position delivers individualized supports within the community and assists clients in navigating complex systems of care.

Position Duties

Client Engagement and Support

Establish trusting relationships through consistent client contact and emotional support
Conduct intake interviews, goal setting, and ongoing service planning with clients
Support clients in developing engagement plans based on their recovery goals
Help clients identify barriers and problem-solve solutions to achieve their goals
Provide motivation and encouragement while celebrating client progress

Case Management and System Navigation

Provide information, referrals, and support in accessing mental health, physical health, social services, and justice-related systems
Assist clients in accessing treatment, attending appointments, and arranging transportation
Support clients in improving social determinants of health such as housing, employment, education, income supports, and disability benefits
Advocate on behalf of clients to ensure access to eligible services and supports
Provide crisis response and intervention as required
Monitor client participation and progress, adapting supports as needed
Maintain accurate and timely client records and documentation

Collaboration and Professional Practice

Work collaboratively with internal staff and external service providers
Participate in professional development and remain informed on best practices in mental health, case management, and community wellness
Integrate Indigenous perspectives and culturally respectful approaches into all services

Terms and Conditions of Employment

CPR and First Aid Certification (or willingness to obtain)
Valid Ontario Class G driver’s license, reliable vehicle, and $1M liability insurance
Annual driver’s abstract
Ability to work flexible hours
Privacy and Confidentiality certification
Successful completion of a Vulnerable Sector Criminal Record Check
Six (6) month probationary period

Work is primarily office-based with regular home visits and community travel required, including travel in winter conditions.

Qualifications

Education and Experience

Completion of a Social Service Worker Program with a minimum of two (2) years’ experience, or an equivalent combination of education and experience
Minimum three (3) years of experience working with adults experiencing serious mental illness and complex life challenges
Minimum two (2) years of direct experience working with Indigenous youth and families in a counselling, social development, or health-related setting

Knowledge and Skills

Knowledge of the health and social challenges facing the Anishnawbek
Understanding of Indigenous cultural values, traditions, and history, with ability to integrate these perspectives into service delivery
Strong crisis response, problem-solving, and advocacy skills
Ability to work independently and collaboratively with minimal supervision
Proficiency in Microsoft Office and electronic documentation systems
Ojibwe language skills considered an asset
