Who We Are:

CarePartners is one of Ontario’s largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 24 nursing clinics.

Overview:

We are currently looking for someone to join our dynamic Nursing Team in the role of Clinical Practice Lead. This person will work closely with our provincial Professional Practice Research & Education (PPRE) team and support clinical standards, orientation, and training. They will collaborate with leaders and help align strategic goals, priorities and initiatives set out and defined by our unique patient populations and Ontario Health Teams (OHTs).

We are looking to fill a Full-Time Temporary 12 month Clinical Practice Lead position in the Erie St. Clair Branch, must be able to work in Lambton - Sarnia counties.

Participation in the Branch On-Call schedule is required, which includes evenings and weekends on a rotational basis.

This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $79,170.00 to $99,528.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What The Role Involves:

Supports, mentors and coaches front line clinical staff while delivering ongoing clinical education designed by the PPRE team.
Works with regional management with respect to Community Health Services, in cooperation with the Ontario Health atHome and OHT if applicable.
Approves, trains and monitors special functions as outlined in the Regulated Health Professions Act and in compliance with Community Health Services policies and practices
Responsible for maintaining quality management procedures and practices within the Nursing team and department.
Promotes and markets Community Health Services and maintains positive public relations.
Builds relationships to enhance local capacity and understanding of system navigation supports, at a local and regional level, to support holistic patient care delivery
Uses new technology e.g. virtual care to support frontline staff and patients to achieve desired care plan goals
Promotes and provides active guidance for workplace Health and Safety.
Performs other duties as required.

What You Bring:

Regulated Health Professional Registered Nurse/ Registered Practical Nurse
Registered with the CNO.
2+ years of community experience in leading exceptional clinical practice in a community care setting
Experience in a formal leadership role considered an asset
Strong palliative experience, an asset
Wound care experience, an asset
Knowledge of adult learning principles, an asset
Strong proficiency in all aspects of Microsoft Office.
Vehicle is required and some occasional travel within Ontario is required
Demonstrated ability to handle highly sensitive and confidential information.
Excellent English verbal and written communication skills
French language skills are an asset
