The Norfolk Family Health Team (NFHT) is a community-based, not-for-profit organization committed to providing accessible, high-quality, and person-centered primary care to individuals and families in Norfolk County. Our services support individuals facing social, economic, and systemic barriers through interprofessional care teams and partnerships.
Our team includes family physicians, nurse practitioners, registered nurses, social workers, a registered dietitian, pharmacist, medical office assistants, a kinesiologist, and an occupational therapist. Additional services such as pharmacy, community lab, ultrasound, and various health and wellness programs are also offered onsite.
Role Overview
We are seeking a permanent full-time Nurse Practitioner (MRP). The NP will deliver comprehensive, team-based primary care to a diverse patient population as part of an interprofessional health team. This role involves autonomous clinical practice, collaboration with physicians and allied health professionals, and a strong focus on chronic disease management, health promotion, and improving access to care within the community.
The successful candidate will be responsible for rostering and managing a panel of approximately 800 patients, providing ongoing primary care and continuity of care. They will contribute to high-quality, person-centered care and may also support specialized programs and initiatives within the organization.

Key Responsibilities
- Provide comprehensive assessments, diagnosis, and treatment within the Nurse Practitioner scope of practice
- Serve as the Most Responsible Provider (MRP) for a rostered panel of approximately 800 patients, ensuring continuity and coordination of care
- Monitor patient response to treatment, including potential side effects of medications
- Order and interpret diagnostic tests to support clinical decision-making
- Manage acute and chronic health conditions using evidence-based approaches
- Prescribe medications and therapeutic interventions in accordance with applicable legislation and professional standards
- Conduct preventative care, screening, immunization, and health maintenance activities
- Collaborate with the interprofessional care team to develop and implement care plans
- Refer patients to specialists, community resources, and other healthcare providers as appropriate
- Support transitions in care and coordinate follow-up after hospitalizations, emergency department visits, and specialist consultations
- Maintain accurate, comprehensive, and timely documentation in the electronic medical record (EMR)
- Participate in clinical quality improvement initiatives, program development, and data collection activities
- Engage in health promotion, patient education, and disease prevention activities
- Contribute to reducing barriers to care and improving access for unattached and vulnerable patient populations
- Provide clinical leadership and support to nursing staff, students, and other team members as appropriate
- Participate in organizational planning, team meetings, and ongoing professional development activities
- Report to the Executive Director and contribute to team and organizational planning
Qualifications
- Master’s Degree in Nursing
- Registered Nurse Certificate of Competence (Extended Class) – College of Nurses of Ontario
- Current CPR for Health Care Providers; ACLS preferred
- Member of NPAO/RNAO and LAP Insurance
- Knowledge of PHIPA, and CNO Standards
- Experience in primary care, chronic disease management, or community health
- Experience working with underserved and marginalized populations
- Familiarity with rural medicine is an asset
Skills and Attributes
- Strong assessment, diagnostic, and treatment planning skills
- Excellent interpersonal, crisis intervention, and communication abilities
- Ability to work independently and as part of an interdisciplinary team
- Experience with EMRs (Practice Solutions preferred)
- Comfortable using interpretation services (e.g., RIO)
- Proficiency in Microsoft Office and virtual platforms (e.g., Teams)
- Interest in program planning and evaluation is an asset
